废用性膝关节挛缩患者日常活动能力现状及影响因素研究

摘　　要：目的了解废用性膝关节挛缩患者日常活动能力现状,并分析其影响因素。方法采用日常活动能力评定量表、徒手肌力评定量表、下肢运动功能评定量表和步行功能评定量表对新疆乌鲁木齐市某三级甲等医院康复科的126例废用性膝关节挛缩患者进行调查,采用多元线性回归分析废用性膝关节挛缩患者日常活动能力的影响因素。结果废用性膝关节挛缩患者的日常活动能力得分为52.50(25.00,80.00)分,不同性别、年龄、病程、医疗费用支付方式的废用性膝关节挛缩患者日常活动能力得分差异有统计学意义(P<0.05);废用性膝关节挛缩患者日常活动能力与肌力、下肢运动功能及步行能力均成正相关(P<0.001);多元线性回归分析结果显示,肌力、运动功能及步行能力是废用性膝关节挛缩患者日常活动能力的影响因素。结论废用性膝关节挛缩患者的日常活动能力处于中度残疾水平,临床康复工作者可从其影响因素入手,根据患者的自身特点强化其肌力、运动功能及步行能力,以期提高患者的日常活动能力。Objective To understand the current status of daily activity ability of patients with disuse knee joint contracture and analyze its influencing factors.Methods A total of 126 patients with disuse knee joint contracture in rehabilitation department of a third-class A hospital in Urumqi were investigated with the activity of daily living scale,manual muscle testing scale,lower limb motor function rating scale and walking function rating scale.Multiple linear regression was used to analyze the influencing factors of daily activity ability of patients with disuse knee joint contracture.Results The score of daily activity ability of disuse knee joint contracture was 52.50(25.00,80.00),which was statistical significance with different gender,age,course of disease and payment method of medical expenses(P<0.05).Patients'daily activity ability was positively correlated with muscle strength,lower limb motor function and walking ability(P<0.001).The results of multiple linear regression analysis showed that muscle strength,motor function and walking ability were the factors affecting the daily activity ability of patients with disuse knee joint contracture.Conclusion The daily activity ability of patients with disuse knee joint contracture is at a moderate level.Clinical rehabilitation workers can refer its influencing factors and strengthen patients'muscle strength,motor function and walking ability according to their own characteristics to improve their daily activity ability.
