摘 要:目的探讨α-硫辛酸联合甲钴胺对糖尿病神经病变患者骨鞘碱性蛋白表达及氧化应激状态的影响.方法选取2019年3月—2020年3月我院收治的200例糖尿病周围神经病变(DPN)患者为研究对象,根据随机数表法分为对照组和观察组,每组100例.两组患者均接受常规治疗,在此基础上,对照组采用甲钴胺治疗,观察组采用α-硫辛酸联合甲钴胺治疗,所有患者均连续治疗1个月.比较两组患者治疗前后的神经病变自觉症状评分(TTS)、神经传导速度[感觉神经传导速度(SNCV)、运动神经传导速度(MNCV)]、骨鞘碱性蛋白水平(MBP)、氧化应激反应指标[血清超氧化物歧化酶(SOD)活性、血清丙二醛(MDA)含量、血清过氧化氢酶(CAT)活性]及不良反应情况.结果治疗后,观察组的各项TSS评分及总评分均明显低于对照组,差异有统计学意义(P<0.05).治疗后,观察组腓总神经、正中神经SNCV、MNCV均快于对照组,MBP水平低于对照组,差异有统计学意义(P<0.05).治疗后,观察组的MDA水平低于对照组,SOD、CAT活性均高于对照组,差异有统计学意义(P<0.05).治疗过程中,两组均未发现严重不良反应.结论α-硫辛酸联合甲钴胺治疗糖尿病周围神经病变的效果显著,能明显改善患者症状和氧化应激状态,降低骨鞘碱性蛋白表达水平,值得临床推广应用.Objective To investigate the effects ofα-lipoic acid combined with mecobalamine on the expression of myelin basic protein and oxidative stress in patients with diabetic neuropathy.Methods A total of 200 patients with diabetic peripheral neuropathy(DPN)admitted to our hospital from March 2019 to March 2020 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 100 patients in each group.Both groups received conventional treatment,on the basis of which,the control group was treated with mecobalamine,and the observation group was treated withα-lipoic acid combined with mecobalamine.All patients were continuously treated for 1 month.The total symptom scores(TTS)of neuropathy,nerve conduction velocity[sensory nerve conduction velocity(SNCV),motor nerve conduction velocity(MNCV)],level of myelin basic protein(MBP),oxidative stress response indexes[serum superoxide dismutase(SOD)activity,serum malondialdehyde(MDA)content,serum catalase(CAT)activity]before and after treatment and the adverse reactions were compared between the two groups.Results After treatment,the TSS scores and total scores of the observation group were significantly lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,the SNCV and MNCV of common peroneal nerve and median nerve in the observation group were faster than those in the control group,and the MBP level was lower than that in the control group,the differences were statistically significant(P<0.05).After treatment,the MDA level of the observation group was lower than that of the control group,the SOD and CAT activities were higher than those of the control group,the differences were statistically significant(P<0.05).During treatment,no serious adverse reactions were found in both groups.Conclusionα-Lipoic acid combined with mecobalamine has a significant effect on the treatment of diabetic peripheral neuropathy,which can significantly improve the
