股骨近端防旋髓内钉内固定与动力髋螺钉内固定治疗股骨粗隆间骨折的临床效果

摘　　要：目的探讨股骨近端防旋髓内钉(proximal femoral nail anti-rotation,PFNA)内固定与动力髋螺钉(dynamic hip screw,DHS)内固定治疗股骨粗隆间骨折的临床效果.方法选取该院2018年1月—2020年6月收治的90例股骨粗隆间骨折患者为研究对象,按随机数字表法分为对照组和观察组,每组45例.对照组采用DHS内固定治疗,观察组采用PFNA内固定治疗.观察两组患者的手术及恢复情况,比较术前及术后的生活质量、关节功能,记录并发症.结果观察组患者的手术时间、负重时间及愈合时间均短于对照组,术中出血量少于对照组,创口小于对照组,组间差异有统计学意义(P<0.05).术后6个月,两组患者的各项生活质量及各项关节功能评分均高于术前,且观察组各项评分均高于对照组,组间差异有统计学意义(P<0.05).观察组的并发症总发生率为11.11%,低于对照组的31.11%,差异有统计学意义(P<0.05).结论PFNA内固定能有效提高股骨粗隆间骨折患者的手术效果,改善其关节功能及生活质量,减少并发症.Objective To investigate the clinical effect of proximal femoral nail anti-rotation(PFNA)internal fixation and dynamic hip screw(DHS)internal fixation in the treatment of intertrochanteric femoral fractures.Methods A total of 90 patients with intertrochanteric fracture of femur admitted to the hospital from January 2018 to June 2020 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 45 patients in each group.The control group was treated with DHS internal fixation,and the observation group was treated with PFNA internal fixation.The operation and recovery of the two groups were observed,the quality of life and joint function before and after surgery were compared,the complications of the two groups were recorded.Results The operation time,loading time and healing time of observation group were shorter than control group,intraoperative blood loss was less than control group,wound was less than control group,the differences between groups were statistically significant(P<0.05).6 months after surgery,the scores of quality of life and joint function in the two groups were higher than those before surgery,and the scores of the observation group were higher than those of the control group,the differences between groups were statistically significant(P<0.05).The total incidence of complications in the observation group was 11.11%,lower than 31.11%in the control group,and the difference was statistically significant(P<0.05).Conclusion PFNA internal fixation can effectively improve the surgical effect of intertrochanteric fracture of femur,improve joint function and quality of life,and reduce complications.
