四妙丸加味治疗下肢深静脉血栓形成患者的疗效及对其D-二聚体与纤维蛋白原的影响

摘　　要：目的探讨对下肢深静脉血栓形成(deep vein thrombosis,DVT)患者采用四妙丸加味治疗的临床效果及对其D-二聚体与纤维蛋白原的影响.方法选择该院2019年1月—2020年11月收治的下肢DVT患者60例,以随机数表法分为两组.对照组(n=30)接受常规西医抗凝、抗血小板治疗,观察组(n=30)在对照组基础上联合四妙丸加味治疗.比较两组临床疗效,分别于治疗前后检测患者的D-二聚体、纤维蛋白原水平以评估两组凝血指标变化情况.结果观察组总有效率100.00%高于对照组的80.00%,差异有统计学意义(P<0.05).治疗前,观察组D-二聚体、纤维蛋白原水平分别为(4.59±0.40)g/L、(2.45±0.82)μg/mL,与对照组相比,组间差异无统计学意义(P>0.05);治疗后,观察组D-二聚体、纤维蛋白原水平分别为(3.42±0.36)g/L、(0.76±0.30)μg/mL,均低于对照组的(3.86±0.39)g/L、(1.28±0.52)μg/mL,组间差异有统计学意义(P<0.05).结论对下肢DVT患者采用四妙丸加味治疗,能够提高临床疗效,改善患者的相关凝血指标,有利于患者康复,值得推广.Objective To investigate the clinical effect of modified Simiao Pill in the treatment of patients with lower extremity deep vein thrombosis(DVT)and its effect on D-dimer and fibrinogen.Methods A total of 60 patients with lower extremity DVT admitted to the hospital from January 2019 to November 2020 were selected and divided into two groups by random number table method.The control group(n=30)received conventional western medicine anticoagulation and antiplatelet therapy,while the observation group(n=30)was combined with modified Simiao Pill for treatment on the basis of the control group.The clinical efficacy of the two groups was compared,and the levels of D-dimer and fibrinogen were detected before and after treatment to evaluate the changes of coagulation indexes in the two groups.Results The total effective rate in the observation group was 100.00%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).Before treatment,the levels of D-dimer and fibrinogen in the observation group were(4.59±0.40)g/L and(2.45±0.82)μg/mL,respectively,compared with the control group,there was no significant difference between the groups(P>0.05).After treatment,the levels of D-dimer and fibrinogen in the observation group were(3.42±0.36)g/L,(0.76±0.30)μg/mL,which were lower than(3.86±0.39)g/L,(1.28±0.52)μg/mL of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion For patients with DVT of the lower extremity,the use of modified Simiao Pill can improve the clinical efficacy,improve the relevant coagulation indexes of the patients,and is beneficial to the recovery of the patients,which is worthy of promotion.
