左卡尼汀对老年维持性血液透析患者微炎症、营养不良和贫血的影响

摘　　要：目的:探讨左卡尼汀对老年维持性血液透析患者微炎症、营养不良和贫血的影响.方法:选择80例老年MHD患者为研究对象,随机分为观察组与对照组,每组40例,对照组予常规MHD治疗及对症处理,观察组在此基础上联合应用左卡尼汀,疗程为3个月.比较两组患者患者治疗前后血清炎症指标(hs-CRP、IL-6及TNF-α)、营养指标(Alb、PAB)及Hb、HCT水平的变化.结果:治疗后,观察组血清hs-CRP、IL-6及TNF-α水平与治疗前比较均明显下降,Alb、PAB明显上升,且观察组优于对照组,差异均有统计学意义;对照组患者上述指标水平与治疗前比较,差异无统计学意义;两组患者血Hb、HCT较治疗前显著上升,且观察组高于对照组,差异均有统计学意义.结论:老年维持性血液透析患者体内常常缺乏左卡尼汀,在常规治疗的基础上加用左卡尼汀可以有效改善患者微炎症及营养状况,有助于贫血的纠正.Objective To investigate the effect of L-carnitine on micro inflammation,malnutrition and anemia in elderly patients with maintenance hemodialysis.Methods 80 elderly patients with MHD were selected and divided into the observation group and the control group randomly,40 cases in each group.The control group was treated with conventional maintenance hemodialysis and symptomatic treatment.The observation group combined with L-carnitine on the basis of this,the course of treatment was 3 months.The changes of serum inflammatory markers(hs-CRP,IL-6 and TNF-α),nutritional indexes(Alb,PAB),Hb and HCT levels were compared between the two groups before and after treatment.Results After treatment,after treatment,the serum hs-CRP,IL-6 and TNF-αin the observation group were significantly lower than those before treatment,and Alb and PAB were obviously increased,and the observation group was better than the control group.The level of the above indexes in the control group was of no statistically significant.The Hb and HCT in both groups increased significantly than before treatment,and the observation group was higher than that of the control group,the difference was of statistically significant.Conclusion L-carnitine is often lacking in the elderly patients with maintenance hemodialysis.On the basis of routine treatment,L-carnitine can effectively improve the patient’s Micro inflammation and nutritional status,and help to correct the anemia.
