中药穴位贴敷治疗慢性阻塞性肺疾病稳定期的疗效

摘　　要：目的:研究中药穴位贴敷治疗慢性阻塞性肺疾病(COPD)稳定期的疗效及对患者血清炎症因子、免疫功能的影响。方法:选取我院2014年2月—2015年6月收治的COPD患者114例,按随机数字表法分为对照组和研究组,各57例。对照组给予舒利迭(1吸/次,bid)治疗,研究组在对照组的基础上给予自制穴位贴敷中药方[分别贴于天突、中府(双侧)、定喘(双侧)、肺俞(双侧),每次贴敷8～12 h,qw]治疗。比较两组治疗效果、血清炎症因子及免疫功能指标水平。结果:研究组治疗总有效率为96.49%,高于对照组的77.19%,差异有统计学意义(P <0.05);治疗后两组干扰素(IFN)-γ、肿瘤坏死因子(TNF)-α水平较治疗前明显下降,且研究组明显低于对照组,差异有统计学意义(P <0.05);治疗后研究组T淋巴细胞亚群CD4^+、CD8^+及CD4^+/CD8^+明显高于对照组,差异有统计学意义(P <0.05)。结论:中药穴位贴敷治疗COPD稳定期疗效确切,可有效调节患者免疫功能,改善炎症状态,值得临床推广。Objective:To study the therapeutic effect of acupoint application of traditional Chinese medicine on stable phase of chronic obstructive pulmonary disease(COPD)and its effect on serum inflammatory factors and immune functions in the patients.Methods:114 patients with COPD admitted to our hospital from February 2014 to June 2015 were selected and randomly divided into a control group and a study group,57 cases each.The patients in the control group were treated with seretide(1 inhalation/time,bid),and the patients in the study group were treated with the acupoint application of self-made Chinese medicine on the basis of the control group[application on Tiantu,Zhongfu(bilateral),Dingchuan(bilaterally),Feishu(bilateral),application once for 8~12 h,qw].The therapeutic effect and the levels of serum inflammatory factors and immune function indexes were compared between the two groups.Results:The total effective rate in the study group was 96.49%,which was higher than 77.19%in the control group,and the difference was statistically significant(P<0.05).The levels of interferon(IFN)-γand tumor necrosis factor(TNF)-αin the two groups after treatment were significantly lower than those before treatment,and the levels in the study group were significantly lower than those in the control group,with statistically significant differences(P<0.05).After treatment,the levels of T lymphocyte subsets CD4+,CD8+and CD4+/CD8+in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The acupoint application of traditional Chinese medicine is effective in the treatment of stable phase of COPD,which can effectively regulate the immune functions of patients and improve inflammatory state,and is worthy of clinical promotion.
