布地奈德与地塞米松对急性喉炎患儿炎症反应及免疫功能的影响

摘　　要：目的:分别探讨布地奈德与地塞米松对急性喉炎患儿炎症反应及免疫功能的改善效果。方法:回顾性分析2015年6月—2018年5月我院收治的116例急性喉炎患儿的临床资料,依据临床用药方案的不同分为两组,各58例。对照组采用地塞米松(0.25～0.5 mg/kg,qd)治疗,观察组采用布地奈德(2 mL,tid)治疗,比较两组炎症因子[C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-(tumor necrosis factor,TNF)-α、白细胞介素(interleukin,IL)]-6水平、免疫功能及症状消失时间。结果:治疗3 d后,观察组CRP、TNF-α、IL-6水平均低于对照组,差异有统计学意义(P <0.05);治疗前后两组CD3+、CD4^+、CD8^+值比较,差异无统计学意义(P> 0.05);观察组哮鸣音、犬吠样咳嗽、声嘶以及呼吸困难消失时间均短于对照组,差异有统计学意义(P <0.05)。结论:与地塞米松比较,布地奈德雾化吸入治疗小儿急性喉炎,可有效缓解炎症反应,缩短临床症状消失时间,且不会对免疫功能产生明显影响。Objective:To investigate the improving effect of budesonide and dexamethasone on inflammatory response and immune function in children with acute laryngitis.Methods:The clinical data of 116 children with acute laryngitis admitted to our hospital from June 2015 to May 2018 were retrospectively analyzed,and the children were divided into two groups according to the different clinical drug use schemes,58 cases each.The children in the control group were treated with dexamethasone(0.25~0.5 mg/kg,qd),while the children in the observation group were treated with budesonide(2 mL,tid).The levels of inflammatory factors[C-reactive protein(CRP),tumor necrosis factor(TNF)-α,interleukin(IL)-6],immune function and disappearance time of symptoms were compared between the two groups.Results:After 3 days of treatment,the levels of CRP,TNF-αand IL-6 in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).There were no statistically significant differences in the values of CD3+,CD4+and CD8+before and after treatment(P>0.05).The disappearance time of wheezing,barking cough,hoarseness and dyspnea in the observation group was shorter than that in the control group,with statistically significant differences(P<0.05).Conclusion:Compared with dexamethasone,budesonide aerosol inhalation can effectively alleviate inflammation response and shorten the disappearance time of clinical symptoms in the treatment of acute laryngitis in children,and has no significant effect on immune function.
