经皮脊柱内镜对腰椎间盘突出患者VAS、JOA及MacNab评分的影响

摘　　要：目的:探讨经皮脊柱内镜治疗对腰椎间盘突出症患者VAS、JOA及MacNab评分的影响.方法:选取2016年10月-2017年12月到本院接受经皮脊柱内镜治疗的腰椎间盘突出症患者80例.其中采用椎间盘镜系统(MED)治疗的对照组患者为34例,采用椎间孔镜下系统(TESSYS)治疗的观察组患者为46例.比较2组患者术前1天、术后3个月、术后6个月视觉模拟评分(VAS);术前1天、术后6个月的JOA临床疗效评分;末次随访的MacNab疗效评分.结果:2组患者术前1天、术后3个月、术后6个月的VAS评分均依次降低,且差异显著(P<0.05).2组患者术后末次随访的JOA评分均显著高于术前(P<0.05).组间比较,2组患者的术前VAS评分和JOA评分没有显著差异(P>0.05),而观察组术后的VAS评分和JOA评分分别低于和高于对照组,差异显著(P<0.05).而2组患者MacNab评分的优良率之间没有显著差异(P>0.05).结论:椎间孔镜下系统(TESSYS)创伤小、疗效确切且恢复快,是治疗椎间盘突出的有效方法,值得临床推广.To investigate the effect of VAS, JOA and MacNab scores of patients with lumbar disc herniation treated withPercutaneous spine endoscope. Methods: 80 patients with lumbar disc herniation to our hospital during Oct. 2016 ~ Dec. 2017 were dividedinto two groups. The 34 patients in research group were treated with micro endo disc system, and the 46 patients in control groupwere treated with transforaminal endoscopic surgery system. Compared between the two groups at the visual analogue scale (VAS), JOAclinical efficacy score and MacNab efficacy score. Results: The VAS scores of two groups at 1 day before surgery, 3 months and 6 monthsafter the operation were decreased, and the difference was significant (P < 0. 05). The JOA scores of the two groups were significantlyhigher than those before operation (P <0. 05). There was no significant difference of VAS score and JOA score between the two groups inpreoperative (P >0. 05). The postoperative VAS score and JOA score of the study group were lower and higher than that of the controlgroup respectively, and the difference was significant (P < 0. 05). And there was no significant difference between the two groups withMacNab score (P > 0. 05). Conclusion: percutaneous spinal endoscope of small trauma, definite curative effect and quick recovery,might be an effective method in treatment of lumbar disc herniation, it was worthy of clinical application.
