摘 要:目的探讨参麦注射液(SM)对体外循环下心脏瓣膜置换术患者的心脏保护作用。方法选择体外循环下行瓣膜置换术患者120例,随机分为参麦组(SM组,60例)和对照组(C组,60例),SM组患者术后静脉输入参麦注射液1ml/kg,C组患者输入等量生理盐水,观察:(1)术后48h血流动力学变化及室性心律失常的发生率;(2)术后48h血管活性药物和利多卡因的使用剂量。结果SM组术后48h平均动脉压(MAP)(102.8±21.0)mmHg、心率(HR)(78.6±12.7)次/min高于C组(98.3±18.8)mmHg、(75.8±12.7)次/min(t=2.04、2.11,均P<0.05),中心静脉压(CVP)(8.78±1.65)mmHg低于C组(9.23±1.88)mmHg(t=2.23,P<0.05);SM组术后48h多巴胺(13.78±6.40)mg/kg、多巴酚丁胺(14.58±5.80)mg/kg、硝普钠(1.87±1.61)mg/kg使用剂量低于C组(18.81±9.36)mg/kg、(19.11±8.37)mg/kg、(2.14±1.54)mg/kg(t=2.12、2.36、2.47,均P<0.05)。结论参麦注射液对体外循环下心脏瓣膜置换术患者的心功能有一定保护作用。Objective To investigate the protective effect of Shenmai injection (SM) on cardiac function of patients undergoing valve replacement under cardio-pulmonary bypass Methods One hundred and twenty patients undergoing valve replacement operation under cardio-pulmonary bypass were randomly divided into the SM group and the control group (60 in each group). Intravenous infusion of 1ml/kg SM was given to patents in SM group after operation, and equal volume of normal saline was given to the controls instead. The following indices were observed: (1) the hemodynamic changes and ventricular premature beat (VPB) in 48h; (2) the dosage of vaso-active drugs and lidocaine used in 48h. Results The mean arterial pressure and heart rate in the SM group after operation were higher than those in the control group (102.8±21.0 vs 98.3±18.8)mmHg, (78.6±12.7 vs 75.8±12.7 bpm), (t=2.04 and 2.11,both P<0.05) , while the central venous pressure was lower than those in the control group (8.78±1.65 vs 9.23±1.88)mmHg, (t=2.23,P<0.05). The dosage of vaso-active drugs dopamine, dobutamine and sodium nitroprusside used during and after operation was lower in the SM group than those in the control group (13.78±6.40 vs 18.81±9.36)mg/kg, (14.58±5.80 vs 19.11±8.37)mg/kg, (1.87±1.61 vs 2.14±1.54)mg/kg, (t=2.12, 2.36 and 2.47, P <0.05). Conclusion SM has certain protective effects on the cardiac function of patients undergoing valve replacement operation under cardio-pulmonary bypass.
