低剂量CTU对上尿路梗阻性疾病的诊断价值

摘　　要：目的:探讨低剂量CTU在上尿路梗阻性疾病中的应用价值。方法:178例研究对象中118例可疑有上尿路梗阻性疾病,60例为正常志愿者。分别在常规剂量平扫、动脉期及静脉期扫描后,行低剂量分泌期扫描,三维重建后,由3名主治医师以上专家进行影像诊断,并追踪病理及治疗结果,对该诊断方法进行真实性和收益评价;同时将扫描总剂量与设想的常规剂量四期扫描总剂量作比较。结果:172例作出明确诊断,6例误诊。诊断敏感性为99.6%(114/118),特异性为96.7%(58/60),粗一致性96.6%(172/178),阳性预测值98.2%(114/116),阴性预测值93.5%(58/60)。常规剂量四期扫描与常规剂量三期扫描加低剂量扫描相比,剂量差异有统计学意义(P<0.01)。结论:常规三期扫描基础结合低剂量CTU既可减少患者受辐射量,又不影响影像诊断,是一项有价值的检查方法。Objective:To make a study on the application value of low dose scanning method applied upper urinary tract obstructive disease.Methods:In all 178 cases,including 118 cases of doubtful upper urinary tract obstructive disease patients and 60 normal cases,after routine dose plain scanning,artery and vein phase scanning,and low dose secretory phase scanning,3D reconstruction,were image-diagnosed by more than three expert physicians,their pathology and treatment results were traced.The truthfulness and efficacy of diagnosis method has been evaluated,at the same time comparing to total dose between scanning a imagined routine dose fourth phase scanning.Results:Of the 178 cases of patient,172 cases were definitely diagnosed,6 cases were msdiagnosed.The diagnoses sensitivity is 99.6%(114/118),specificity is 96.7%(58/60),crude agreement is 96.6%(172/178),masculine forecast value is 98.2%(114/116),negative forecast value is 93.5%(58/60),efficacy value is high.Through comparison between routine dose fourth phase scanning and routine dose third phase scanning plus low dose scanning,dose differences have statistical significance(P<0.01).Conclusion:The combination of routine dose third phase scanning and low dose CTU can reduce the radiation quantity accepted by patients,but dose not effect the diagnosis results.It is a valuable diagnostic method.
