摘 要:目的:检测2型糖尿病患者外周血Th1/Th2淋巴细胞及血清高敏c-反应蛋白(hs-CRP)的水平,以探讨免疫炎症与2型糖尿病的关系。方法:运用三色荧光标记法流式细胞术检测外周血中Th1/Th2淋巴细胞,用ELISA法测定hs-crp。结果:①2型糖尿病患者Th1数,Th1/Th2比值与正常对照组相比明显降低(P<0.01),Th2数与正常对照组无明显差异。2型糖尿病患者外周血Th1淋巴细胞和HbA1c呈负相关关系(r=-0.688,P<0.01)。②2型糖尿病患者血清hs-CRP与正常对照组相比明显升高(P<0.05)。糖尿病患者血清hs-CRP浓度与胰岛素敏感性指数呈显著负相关(r=-0.38,P<0.01),与甘油三酯(r=0.33,P<0.05)、体重指数(r=0.42,P<0.05)和空腹血糖(r=0.46,P<0.05)有正相关关系。结论:2型糖尿病患者存在Th1/Th2淋巴细胞的失衡,血清hs-CRP水平与胰岛素抵抗程度有显著正相关,免疫炎症可能参与了2型糖尿病的发病。Objective:To investigate the clinical significance of Th1/Th2 balance of peripheral blood Th cells and of serum high-sensitivity C-Reactive Protein(hs-CRP) in type 2 diabetic patients.Methods:Th1/Th2 cells were determined by using the tri-colored fluorescence labeled flow cytometry serum hs-CRP concentrations were determined by ELISA in type 2 diabetic patients (n=60) and the controls (n=25).Results:(1)Th1 cells and Th1/Th2 ratios were significantly reduced in type 2 diabetic patients than the controls (P< 0.01 ). There were no any difference in Th2 cells between diabetic patients and the controls (P> 0.05 ). Th1 cells negatively correlated with HbA1c (r=-0.688 P< 0.01 ). (2) The serum hs-CRP levels were significantly higher in type 2 diabetic patients than in normal controls (P< 0.01 ). There was a significant negative correlation between the serum hs-CRP concentrations and IAI (r=-0.38,P<0.01). hs-CRP was positively associated with Triglycerides (r=0.33 P< 0.05), and BMI (r=0.42,P<0.05), and fasting blood glucose (r=0.46,P< 0.05).Conclusions:Th1/Th2 unbalance of peripheral Th cells and high levels of hs-CRP might result in the course of type 2 diabetes mellitus. Immune-Inflammation may play an important role in pathogenesis of type 2 diabetes mellitus.
