摘 要:目的探讨心瓣膜的损害程度及心功能状态与血尿酸关系。方法测定心瓣膜病383例,单瓣或联合瓣膜病变、心功能状态、是否合并肺动脉高压患者的血尿酸和尿素氮浓度。结果联合瓣膜病变者血尿酸浓度明显高于单瓣膜病变组。瓣膜病患者血尿酸浓度随心功能的恶化而增高(r=0.338,P=0.000),心功能Ⅰ~Ⅱ级与Ⅲ级患者比较,血尿酸浓度差异有统计学意义(P=0.000),但尿素氮浓度差异无统计学意义,心功能Ⅲ级与Ⅳ级患者比较,血尿酸浓度和尿素氮浓度差异有统计学意义(P=0.014,P=0.013)。心瓣膜病合并有肺动脉高压的血尿酸浓度比无肺动脉高压组高(P=0.045)。结论心瓣膜病患者血尿酸浓度与瓣膜损害程度、心功能恶化程度及合并肺动脉高压有关,是心瓣膜病者病情严重程度的重要生化指标。Objectives To explore the clinical significance of serum uric acid level in patients with cardiac valvular disease and heart failure.Methods The data consisted of 383 patients with cardiac valvular disease.Serum uric acid and BUN were assessed in following groups:patients with single valvular and multivalvular heart disease;patients with different severity of heart failure;with or without pulmonary hypertension.Results The serum uric acid level of multivalvular heart disease was higher than single valvular.The concentration of serum uric acid in patients with heart function class Ⅰ-Ⅱ also was higher than class Ⅲ(P<0.01).The concentration of serum uric acid and BUN in patients between heart function class Ⅲ and IV were significantly deferent(P<0.05).The serum uric acid level of cardiac valvular disease with pulmonary hypertension was higher than that without one.Conclusions Serum uric acid level is closely related to severity of cardiac valvular hurt and heart failure and pulmonary hypertension.It is an index of severity of cardiac valvular disease.
