摘  要:目的提高对多发性硬化(多发性硬化)的影像诊断水平。方法对50例临床确诊的多发性硬化患者的CT和MRI影像资料进行总结分析。结果50例多发性硬化在MRI上均发现异常,病灶为T1WI呈等或稍低信号,T2WI呈高信号,位于双侧侧脑室周围白质36例,半卵圆中心26例,脑干15例,小脑5例,颈髓3例,病灶大小不一,单发或多发。50例CT扫描26例表现为正常,23例见脑室周围和半卵圆中心单发或多发不规则低密度影,1例仅表现为脑萎缩。结论MRI能发现许多CT不能显示的病灶,其敏感性和特异性均高于CT;特别是MRI能显示多发性硬化的“垂直脱髓鞘征”特征性表现,是诊断多发性硬化最好的影像学检查方法。Objective To improve diagnosis of multiple sclerosis (MS). Methods 50 patients that had been clinically diagnosed as MS were retrospectively analyzed with their CT and MRI data. Results All 50 patients were found positive for lesions of MS using MRI, presenting iso- or slightly hypointensity on T1WI and hyperintensity on T2WI. The lesions were situated in white matter surrounding both lateral ventricles (36), centrum semiovale (26), brain stem (15) and cervical spinal (3). The lesions were diversified in size single or multiple. On CT, 26 patients were determined normal by CT, and CT found single or multiple hypointensities in 23 patients whose lesions were situated around the brain ventricles or centrum semiovale, 1 patient only displayed brain atrophy. Conclusion MRI is superior to CT being capable of finding many lesions that CT cannot, enjoying higher sensitivity and specificity. What’s more, MRI is able to detect vertical demyelination, which is characteristic sign of MS. Thus, MRI is the best method to diagnose MS.
