急性颅脑外伤手术患者血清NSE的测定及意义

摘　　要：目的:测定急性颅脑外伤手术患者血清神经元特异性烯醇化酶(NSE)的含量,探讨其与患者格拉斯哥(Glasgow)评分和6个月后简易智能量表(MMSE)评分的相关性。方法:10例泌尿系统手术的非颅脑外伤患者为对照组,测定术前及手术结束时血清NSE含量。另30例急性颅脑外伤手术患者为脑外伤组,术前行Glasgow评分,并术前及手术结束时测定血清NSE含量,6个月后随访时,以MMSE量表对其中23例患者进行评分。结果:脑外伤组术前NSE含量显著高于对照组(P<0.01),Glasgow评分越低的患者术前NSE含量越高(P<0.05);手术结束时患者NSE含量较术前升高,其浓度与6个月后患者MMSE评分呈显著负相关(P<0.05)。结论:急性颅脑外伤患者血清NSE含量升高;外伤越严重,其含量也越高,手术结束时患者血清NSE含量可用作判断患者预后的重要指标。Objective:To determine the concentration of serum NSE in patients with brain trauma undergoing neurosurgery and its relation with Glasgow score and MMSE score 6 month later.Methods:Ten patients undergoing urological operation without brain trauma served as control group.The concentration of serum NSE of these patients was determined by enzyme-linked immunoadsorbent assay before and after operation.In 30 patients with acute brain trauma undergoing neurosurgery as brain trauma group,the Glasgow score before operation was evaluated,serum NSE concentration was measured before and after operation,and 6 months later during follow-up by using MMSE,23 out of 30 patients with brain trauma were also estimated.Results:The preoperative serum NSE concentration in brain trauma group was significantly higher than that in control group(P<0.01).The Glasgow score was significantly negatively correlated with the serum NSE concentration(P<0.05).The serum NSE concentration after operation was higher than that before operation,and it was significantly negatively correlated with the MMSE score of the patients 6 month later(P<0.05).Conclusion: The serum NSE concentration was increased in the patients with acute brain trauma.The severe the trauma was,the higher the serum NSE concentration was.After operation,the NSE concentration could be used as an important indicator to evaluate the prognosis of the patients.
