心律失常患者的冠状动脉造影结果对比分析

摘　　要：目的:观察心律失常患者的冠状动脉造影(CAG)阳性率。方法:99例患者,平均年龄55.1±7.0岁。在①年龄≥50岁的与<50岁的;②心律失常≥2种的与1种的;③有冠心病(CAD)危险因素的与无CAD危险因素的;④心电图有缺血型的ST-T改变(简称ST-T改变)的与无ST-T改变的;⑤心绞痛症状典型的与不典型的之间进行比较。结果:≥50岁者CAG阳性率为60.0%(45/75),<50岁者为37.5%(9/24);≥2种心律失常者CAG阳性率为61.5%(8/13),仅有1种心律失常者为53.5%(46/86);此两组对比均无明显差异(P>0.05)。有CAD危险因素者CAG阳性率为72.7%(24/33),无CAD危险因素者45.5%(30/66);有ST-T改变者为81.8%(18/22),无ST-T改变者为46.8%(36/77);心绞痛症状典型者为92.2%(47/51),不典型者为14.6%(7/48);三组对比差异显著(前二组P<0.05,后一组P<0.001。结论:心律失常患者伴有CAD危险因素、ST-T改变、典型心绞痛症状的,有较高的CAD阳性率。如诊断有疑问,应进行CAG检查。Objective: To observe the positive rate of CAG for arrhythmia patients.Methods:The 99 arrhythmia patients was divided the latter groups: ①the age ≥ 50 year-old vs <50 year-old;②have≥2 classes arrhythmia vs 1 class arrhythmia;③have the risk factor of CAD vs without the risk factor of CAD;④electrocardiogram(ECG) have ST-T alteration(ST-T alteration) vs without ST-T alteration;⑤have typical angina symptom vs without typical angina symptom.Results:The positive rate was 60.0%(45/75) for age≥50year-old patients,was 34.4%(9/24) for<50 year-old patients;The positive rate was 61.5%(8/13) for 2 classes arrhythmia patients,was 53.5%((46/86) for 1 classes arrhythmia patients;this two group contrast without marked difference(p>0.05).The positive rete was 72.7%(24/33) for patients who have CAD risk factor,was 45.5%(30/66) for patients who without CAD risk factor;the positive rate was 81.8%(36/77) for patients whose ECG without alteration;the positive rate was 92.2%(47/51) who have typical angina symptom,was 14.6%(7/48) who without typical angina symptom;this three group contrast have marked difference(the two former group P<0.05;the latter group P<0.001).Conclusion:Arrhythmia patients who have the risk factor of CAD、ST-T alternation、typical angina symptom,who have more positive rate of CAD.If diagnose have doubt,should chuck-up CAG.
