摘  要:目的分析新型铜蒸气激光为光源的光动力学疗法治疗鲜红斑痣的有效性及各种因素对疗效的影响。方法鲜红斑痣患者136例。静脉推注血卟啉单甲醚(HMME)3.5~5.0mg/kg、5min内行铜蒸气激光照射。激光功率密度80~100mW/m2,光斑直径8~9cm,照射时间20~40min,能量密度120~240J/cm2。治疗随访1个月以上,分析疗效与患者年龄、鲜红斑痣分型、病变部位的关系。结果136例中总有效率94.8%,完全治愈11.8%,基本治愈33.8%,好转率35.3%,改善率13.9%,无效率5.2%。经Spearman相关分析疗效与患者年龄、病变分型有相关关系,经KruskalWallis检验不同病变部位的疗效差异有显著意义(P=0.002)。结论铜蒸气激光为光源的光动力学疗法治疗鲜红斑痣安全有效,患者年龄、病变类型及病变部位与治疗效果有关。Objective To study the curative effect of photodynamic therapy (PDT) for port wine stains (PWS) using a new type copper vapor laser,and to detect the relationship of curative effect with patient's age, type and location of PWS, respectively. Methods The patients with different kinds of PWS were treated by photodynamic therapy using the new type copper vapor laser and the second generation photosensitizer HMME. Results In 136 cases, the general effective rate was 94.8%, while there was 11.8% evaluated as excellent, 33.8% as good, 35.3% as fair, 13.9% as poor and only 5.2% was evaluated as no effect. Conclusions PDT with copper vapor laser is effective and safe for the treatment of PWS and the curative effect is associated with patient’s age, type and location of PWS.
