摘 要:目的:观察依诺肝素对进展性脑梗死的预 防、治疗作用与安全性。方法:将起病24h以内的 312例急性脑梗死病人随机分为2组,常规治疗组 150例用常规治疗,依诺肝素组162例除常规治疗 外给依诺肝素0.4mL(相当于4000AxaIU),sc,q 12h×10d,治疗后观察2组进展性脑梗死的发病 率,治疗前后检测进展性脑梗死病人纤溶酶原活性、 凝血因子Ⅹ活性及脑梗死病灶体积的变化。治疗前 后评估进展性脑梗死病人的神经功能缺损评分 (CSS)。结果:依诺肝素组发生进展性脑梗死23 例,常规治疗组为35例(P<0.05),依诺肝素组进 展性脑梗死病人的神经功能及脑梗死病灶的进展程 度明显低于常规治疗组(P<0.01),依诺肝素组进 展性脑梗死病人的纤溶酶原活性和凝血因子X活 性治疗后明显低于治疗前(P<0.01),治疗期间不 良反应轻微。结论:依诺肝素治疗能降低进展性脑 梗死的发病率,有利于进展性脑梗死病人神经功能 恢复。AIM: To observe the therapy, prevention and the safety of enoxaparin for the treatment of progressive cerebral infarction. METHODS: Three hundred and twelve patients of acute cerebral infarction less than 24 h of the onset were randomly divided into two groups. The routine therapy group 150 patients were treated under the general way. The enoxaparin group 162 patients were given enoxaparin 0.4 mL (balancing 4 000 AxaIU), sc, q 12 h×10 d in addition to the general treatment. The incidence of progressive cerebral infarction in two groups were closely observed after wards. The change of plasminogen's activity and coagulation factor X's activity and infarctional focal volume of progressive cerebral infarction were measured minutely before and after treatment. The nerve function deficits of patients of progressive cerebral infarction were evaluated by China Stroke Score (CSS) before and after treatment. RESULTS: Twenty three patients became progressive cerebral infarction in the enoxaparin group, and in the routine therapeutic group 35 patients became (P<0.05). The progressive degree of nerve function and infarctional focal volume obliously were lower in the enoxaparin group than in the routine therapeutic group (P< 0.01). The activity of plasminogen and coagulation factor Ⅹ of progressive cerebral infarction in enoxaparin group were obviously lower than before treatment (P<0.01). Only slight adverse reaction occurred during the whole course of treatment. CONCLUSION: Enoxaparin can decrease the incidence of progressive cerebral infarction and be benefitial to neurological functional recovery of patients getting progressive cerebral infarction.
