1600例腹腔镜胆囊切除术无胆管损伤的临床体会

摘　　要：目的探讨如何预防腹腔镜胆囊切除术中胆管损伤。方法回顾分析2004年5月～2007年4月1600例腹腔镜胆囊切除术使用胆囊壶腹后开窗法的临床资料。结果1例有上腹部手术史和1例有急性胰腺炎发作史者因腹膜腔粘连严重、气体膨胀困难而中转开腹切除胆囊。2例术中快速冷冻切片提示意外胆囊癌,中转开腹行胆囊癌根治术,1600例中无1例胆管损伤及其他并发症。术后平均住院日3.5d。结论胆囊壶腹后开窗法可帮助术中辨认肝外胆管的解剖结构及变异,在有效减少胆管损伤方面有一定作用。[Objective] To study how to avoid the biliary duct injury caused by laparoscopic cholecystectomy (LC) using gallbladder posterior ampulla fenestration (GPAF). [Methods] 1600 patients received LC from 2004 to 2007 were retrospectively reviewed. [Results] 2 of the patients were converted to open cholecystectomy (OC) due to severe adhesion of peritoneal cavity. 2 of them were converted to open operation due to accident gallbladder cancer. No biliary duct injury and other complications occurred in all the cases...
