体位干预对C_3F_8气体或硅油充填眼康复的影响

摘　　要：目的 探讨护理干预对玻璃体切割、眼内C3F8气体或硅油充填术后病人的影响。方法将1998年3月至2001年3月行玻璃体切割、眼内C3F8气体或硅油充填术病人168例(179眼)作为对照组,实施常规护理;2003年1-8月该类病人160例(179眼)作为干预组,在常规护理基础上采用术前训练体位、手术后辅导和监督体位,以及指导病人采用多种体位变化和适当活动等方法实施体位干预。记录并统计两组病人手术后眼部及全身反应发生情况。结果无晶体眼病人术后出现充填物进入前房发生率,干预组与对照组比较,差异有显著性意义(P<0.05);病人术后眼压及视网膜复位情况比较,差异有显著性意义(均P<0.05);年龄≥60岁病人发生血压升高、食欲不振、肢体活动障碍及受压部位异常情况,干预组与对照组比较,差异有显著性意义(均P<0.05或P<0.01)。结论对玻璃体切割眼内充填C3F8气体或硅油充填术病人实施体位干预能有效降低术后眼部并发症,也可减少全身异常情况发生。Objective To investigate thc effect of positional change in nursing care on vitrectomized patients tamponade with silicon oil or C3 F8. Method One hundred and sixty vitrectomized patients with tamponades (179 eyes involved) from January to August in 2003 in our hospital under special positional nursing served as experimental group and 168 vitrectomized patients with tamponades (179 eyes involved) in 1998-2001 in our hospital as control group, in which routine nursing care was given. In the experimental group, the preoperative positional training and postoperative guidance were given and the position was monitored. Results The number of the aphakic patients whose tamponades got into in anterior chamber in the experimental group was significantly lower than that in control group after operation (P<0. 05). After the operation. there was significant difference in eye pressure and retinal re-attachment (F<0. 05 for both). The number of old patients (≥60) with abnormal physical condition in the experimental group was significantly lower than that in control group (P<0. 05). Conclusion Positional nursing care for vitrectomized patients tamponade with silicon oil or C3F8 can reduce the complications of vitrectomy and the abnormal physical condition.
