四肢手术应用止血带对病人血压脉搏的影响

摘　　要：目的 了解骨科四肢手术过程中应用止血带对病人血压脉搏的影响。方法 对72例病人,分别观察止血带放松前、放松后1、3、5、10 min的血压、脉搏及全身情况。结果 下肢手术者放松止血带1、3 min时血压明显下降,心率显著加速(均P<0.01),5-10 min后逐渐恢复;发生“止血带休克”3例,经对症处理10-20 min恢复正常。上肢手术者血压、脉搏变化不显著(均P>0.05)。结论 放松止血带后可导致血压下降、脉率加快,以下肢手术者为著。提出在放松止血带前采取加快补液速度,对症适量用药,尽量减缓松止血带速度等措施,以预防不良反应的发生。Objective To observe the effect of pneumatic tourniquet on the blood pressure and pulse of orthopaedic patients. Methods Seventy-two cases were observed before and after deflating the tourniquets (1,3,5,10 min). Results The blood pressure and the pulse were affected observably on the operations of lower extremities (all P<0. 01). Three cases had 'tourniquet shock', After 10-20 min, all of them returned to normal by using appropriate therapy. The effect on the super extremities was not observable(P>0. 05). Conclusion It is effective to prevent the side-effect by speeding up transfusion, using appropriate medicine and deflating the tourniquet.
