摘 要:目的探讨严重烧伤病人早期肠内营养实施的方案。方法将 4 0例严重烧伤病人随机分为观察组 (18例 )和对照组 (2 2例 ) ,对照组按常规方法实施肠内营养 ;观察组入院后 1~ 2h内予以少量流质饮食 ;肠内营养制剂为整蛋白与氨基酸类合理搭配 ;营养液输注速度有计划地逐步增加。记录两组胃肠道症状如恶心、呕吐、腹泻、腹胀、胃肠道出血发生情况。结果腹泻发生率观察组明显少于对照组 (P <0 .0 1)。结论严重烧伤后早期肠内营养安全可行 ;应尽可能早地进行肠内营养 ;及时调整肠内营养液的配方、输注速度 。Objective To explore a better way of early enteral nutrition in patients with sever burns.Methods Forty severe burn patients were randomly divided into two groups:observation group (n=18) and control group(n=22). The control group was routinely given the enteral nutrition. In the observation group, small amount of fluid food was given within 1~2 hour after admission. The preparation for enteral nutrition contained right mix of whole proteins and amino acids. The rate of infusion was increased gradually under controlling. The symptoms of digestive tract of the patients were observed on daily basis, including nausea, vomiting, diarrhea, tympanites, and gastrointestinal hemorrhage . Results In observation group the rate of diarrhea was much lower than that in control group ( P< 0.01). Conclusions The early enteral nutrition is feasible with patients with sever burns and should be started as soon as possible. Prompt adjustment of ingredients of nutrition liquid, infusion rate can help lower the gastrointestinal complications.
