广东省各市首例传染性非典型肺炎病例流行病学分析

摘　　要：目的 分析广东省各市首例传染性非典型肺炎病例(IAP,WHO称为SARS)的流行病学特征,为追溯传染源提供科学依据。方法 采用统一的个案调查表对广东省各市IAP首发病例进行调查,对其密切接触者也进行流行病学调查,分析各市首发病例流行病学特征、续发病例情况及各市首发病例之间的联系。结果 2002年11月16日至2003年4月16日,广东省有13个地级市报告发生(依序为佛山、河源、中山、江门、广州、深圳、肇庆、东莞、汕头、惠州、汕尾、湛江、韶关等市)。对13个市的“首发”病例进行分析:13例首发病例中,厨师和干部各有3例,农民、退休人员、工人各2例、商业人员1例;男女比例为1:0.6,年龄在18～84岁之间,30～50岁组占77％(10/13);死亡4例。2000年11月至2003年1月期间,7个市报告的首发病例中,6例为本市感染病例,1例怀疑为从广州输入病例,均无明确同类患者接触史,其中有6例引起二代或三代病例;2003年2月无新发市;2003年3月至4月16日,6市报告新发病例,2例为本市感染发病,3例感染地为香港、1例感染地为广州,5例有明确的同类患者密切接触史,只有2例引起第2代病例。结论 未发现13个首发病例间存在相互传播的线索。从13个市追溯到的可能源头集中在珠三角的6个市和香港地区。报告病例早期,由于对IAP认识不足。Objective To analyze the epidemiological features of the index cases of infectious atypical pneumonia(IAP) occurred in different cities among Guangdong province for providing the evidence of tracing back the reservoir. Methods Standard individual case inventory was adopted to conduct a investigation of index cases and their close contact persons in Guangdong province. Epidemiological characteristics, clinic course, secondary case status and the links among index cases were analyzed. Results The index cases which reported before January 31,2003 were mostly infected in local areas, with no distinguished contact history and the second or the third cases out coming. The index cases reported after February 1,2003 were mostly imported cases with distinguished contact history. Mostcases were junior adult (7/13)with high fatality rate. The percentage of gender was 1:0.6 between male and female. Conclusion No clues of inter - transmission could be found among the index cases. The index cases could be traced back occurring in six cities of Pearl River delta region and the Hong Kong Special Administrative Region so far. Quite a lot of number of the second cases and the third cases were happened in the early period for lacking enough recognition of IAP.
