60例老年性前列腺增生症X线立体定向放射治疗研究

摘　　要：目的报告60例老年性良性前列腺增生症(BPH)X线立体定向放射治疗(SRT)后的临床及影像学变化,讨论SRT对老年性前列腺良性增生症的治疗作用。方法利用CRT-98立体定向放射治疗系统,常规进行体模固定,以3mm或5mm层厚CT扫描,将影像信息输入治疗计划系统(3DTPS),图像重建,制定治疗计划,采用多角度,非共面,单靶区旋转照射,分割5-6次,以80%等剂量曲线覆盖增生(肥大)的前列腺腺体边缘,治疗总剂量20～25Gy。结果全组60例获得8～48个月的随访。治疗后1个月60例患者均有不同程度的尿频、尿急、尿痛症状,B超复查残余尿30±12ml,较治疗前明显减少;3个月后复查58例尿频、尿急、尿痛症状消失占96.7%,2例有轻度尿频占3.3%,B超复查10例有少量残余尿,CT复查60例患者均显示腺体缩小或恢复正常,国际前列腺症状评分(IPSS)由治疗前28.6±3.0降至9.2±2.0。结论SRT治疗BPH能够对增生的腺体提供一定的治疗剂量,使过度增生的腺体细胞变性坏死,达到消除或缓解尿梗阻症状的目的。Objective To review the changes of clinical and image in 60 cases with senile benign prostate-hyperplasia (BPH) treated with stereotactif radiotherapy and analyzed the treatment effectiveness of prostate-hyperplasia.Methods CRT-98 physical X-knife system was used from Oct2000 to Oct2004.First the patient fixed in the phantoms,then imaged by 3mm or 5mm intensify CT,the image was put into the computer workshop and was 3- dimensionally rebuiltThe techniques of multi-angle,non-con-area,single-target center were taken edvantage.The 80% iso-dose curve covered the whole hyperplastic prostate and the total effect was equal to froro 20Gy to 25Gy in average radiation. The whole dose was divided into 5 to 6.Results All 60 cases were observed from 8 months to 48 months.l month after therapy,all 60 cases were represented more or less symptoms of frequent micturition,urgent micturition or dysuria etc.The trine that remained was approximately 30±12ml examined by B ultrasonic,that was obviously reduced:3months after therapy,the symptoms of frequent micturition,urgent mieturition or dysuia were disappeared in 58 cases that was 96.7%of the whole cases,and 2 cases that was only 3.3% also represented frequent micturition.And 10 cases were show a little mine remain examined by Bltrasonic,examined by CT,all 60 cases displayed that the prostate was reduce or coming back.IPSS was reduce from28.64±3.0 to 9.2±2.0.Conclusion Stereotactic radiotherapy can heighten the radioactive dose for the hyperplastic prostate cells,may make the hyperplastic prostate cells denaturalized and putrescence,reach the result that was clear-up or reduce the symptom of urine blocking.
