延长氯米芬用药时间并口服达英-35治疗耐氯米芬的无排卵患者疗效分析

摘　　要：目的观察常规口服氯米芬促排卵失败的扫女,服避孕药达英-35配合延长氯米芬服用时间,促排卵的治疗效果.方法115例患者在采用传统方法促排卵连续3个周期无排卵后,改服达英-35三个周期以上。从停药后月经第5天服用氯米芬(50mg/d)促排卵,阴道B超(TVS)监测排卵经过以及子宫内膜厚度,至致出现优势卵泡停服.结果115例患者在服用氯米芬5d后,有38例出现优势卵泡,36例成功排卵,3例出现卵泡黄素化未破裂综合症(LUFS);延长服药时间后又有35例成功排卵,18例出现LUFS.14例有卵泡形成,但未达到优势卵泡即闭镇,9例无卵泡生长。结论延长氯米芬用药时间并口服达英-35治疗耐氯米芬的无排卵患者,是一种值得推广的有效促排卵措施。Objective To evaluate the effectiveness of extended clomiphene citrate (CC) with Diane-35(Compound cyproterone acetate tablet)for women who had failed to ovulate with conventional cc regimen.Methods I15 patients who had failed to ovulate with 3 cycles of traditional methods switched to Diane-35 for at least 3 cycles.Then on 5th day of withdrawal of Diane-35, CC is taken 50mg/d,until the super-follicle is observed.Ovulation and thickness of endometrinm were surveyed with transvaginal ultrasound (TVS).Results In these 115 patients 38 had super-follicle after taking CC for five days,among whom, 36 ovulated and 3 developed luteinized Unruptured Follicle syndrome (LUFS).With extended CC therapy,35 more patients ovulated and 18 more had LUFS.14 patients suffered from follicle atresia before the formation of surer-follicle,while another 9patients,follicle failed to developed at all.Conclusion Anovulatory patients resistant to CC alone,have hige rates of ovulation after treatment with extend CC and Diane-35.
