周围动脉狭窄和栓塞的介入治疗临床研究

摘　　要：目的研究用选择性介入留管灌注治疗周围动脉狭窄或闭塞性疾病的临床应用。方法40例周围动脉狭窄或闭塞的病人随机分为两组.介入组采用动脉穿刺并留管灌注尿激酶,对照组静点尿激酶、疗程为10天。疗前疗后通过多普勒超声、数字减影血管造影的检测进行疗效对比。结果介入组临床痊愈55.5%,显效34.5%,好转10%,总有效率100%。对照组临床痊愈36.7%,显效16.7%,好转40%,总有效率90%.结论选择性介入留管灌注能迅速改善肢体动脉供血,促进血栓溶解和血管再通.对坏疽性肢体有明显镇痛和解除血管痉挛的作用,使诊治融为一体,并提出留管规律性时限为2周内。OBJECTIVE-To study the value of the selective inter-ventional indwelling catheter perfusion in the diagnosis and treatment of the stenosis and obliteration of the perepberal artery.METHODS:40 cases with stenotis and obliterative disea-ses were divided in to two groups according tu random alloca-tion.Arteriopuncture was performed and perfused vrokinase th-rough indwelling catheter in the interventional group, Uroki-nase was used by intravenous drip in the control group,both course of treatment were 10 days.Compared the therapeutical effects with ultrasonic doppler and Digital subtraction Angio-graphy before and after treatment.RESULTS:In the intervene-tional group,cases were cure with rate of 55.5%,cases were obviously effective with rate of 34.5%,and cases with better than before with rate of 10%.The total effective rate were 100%.In the control group,cases were cure 36.7%,cases were obviously effective 16.7%,and cases were better than before 40%,the total effective rate were 90%.CONLUSION:The selective intervenetional indwelling catheter perfusion could improve blood supply of extremital artery rapidly,promote thrombi dissolution and revascularisation.It also had the effect of analgesia on gangrenous extremity and relieved spasm of vessels:It made diagnosis and treatment together,and proposed the regular limit time of indwelling catheter was in 2 weeks.
