乳管镜下乳管内肿瘤的诊断及手术定位

摘　　要：目的探讨乳管镜和乳腺定位导丝对乳管内肿瘤的诊断和定位价值。方法2006年7月至今,对28例Ⅰ、Ⅱ级导管内肿瘤患者行乳管镜检明确肿瘤及位置,经乳管镜置入乳腺定位导丝,确认其倒钩将肿块钩住。术中选择乳晕边缘小切口,沿定位导丝切除乳头下至距肿瘤远端2cm病变乳管及瘤体。剖开乳管定位瘤体后送病理检查。结果28例患者中发现早期乳腺癌2例(7.1%),乳管内乳头状瘤20例(71.4%),乳头状瘤病6例(21.4%)。结论乳管内镜能直接观察乳头溢液患者乳管内情况及肿块,提高乳头溢液患者病因诊断的准确性,而在其协助下经溢液乳孔放置乳腺定位导丝,能够避免手术盲目性,为病灶的检出提供可靠的帮助。【Objective】To evaluate the applicative value of fiberoptic ductoscopy (FDS) in abnormal nipple dis- charge. 【Methods】 A total of 28 patients with nipple discharge caused by intraductal tumors were examined by fiberoptic ductoscopy to determined the location of lesion, and give them further surgical treatment to confirm their pathological diagnosis. 【Results】Of the 28 cases, early breast cancer were found in 2 cases (7.1%), papilloma in 20 cases (71.4%), papillomatosis in 6 cases (21.4%). 【Conclusions】Intrad...
