经腹膜后腹腔镜手术治疗468例单纯肾囊肿疗效观察

摘　　要：目的:分析经腹膜后腹腔镜,采用超声刀对单纯肾囊肿行去顶减压术的疗效、安全性和临床价值。方法:对468例经腹膜后腹腔镜肾囊肿去顶减压临床资料进行回顾分析。手术中,通过气腹使后腹膜膨胀,通过腰大肌、腹膜返折、膈肌脚等标志找到并暴露肾囊肿,超声刀距肾实质0.5～1cm将囊壁切除去顶。结果:经后腹膜入路468例患者肾囊肿顺利切除去顶,其中左侧囊肿183例(39.1%),右侧囊肿164例(35.0%),双侧101例(21.6%),多囊肾20例(4.27%)。平均手术时间53.18±0.95分,平均术中出血量6.28±0.33mL,15例出现术后并发症。平均住院天数8.97±0.12天,平均术后住院天数4.90±0.07天。结论:经腹膜后腹腔镜肾囊肿去顶减压是一种安全、有效且可行的治疗方式。Objective:To analyse the efficacy,safety and feasibility of retroperitoneal laparoscopic decortication in the treatment of renal cyst by using ultrasonic scalpel.Methods:Records of 468 patients who underwent laparoscopic decortication of simple renal cysts,performed with ultrasonic scalpel,were retrospectively reviewed.Results:With renal cysts removed,the operations were successfully accomplished in all of the 468 patients.One-hundred eight-three cases in the left(39.1%),164 cases in the right(35.0%)and 101...
