摘  要:目的 探讨血清同型半胱氨酸水平对颈动脉结构与功能的影响。方法 研究对象共 81例包括冠心病患者 5 7例 (年龄 5 8岁± 8岁 )和正常人 2 4例 (年龄 5 5岁± 9岁 )均经选择性冠状动脉造影证实。应用惠普Sonos 5 5 0 0型超声诊断仪测定及计算颈动脉内膜 -中层厚度、斑块积分等颈动脉指标。血清同型半胱氨酸水平应用高效液相色谱法测定。应用SAS软件包进行统计学处理。结果 冠心病组同型半胱氨酸水平明显高于正常对照组 [(17.0 6± 4.5 4)μmol/Lvs3.2 1μmol/L ,P <0 .0 1]。颈动脉内膜 -中层厚度、斑块积分、僵硬度 β和Peterson s弹性指数EP冠心病组均明显大于对照组 (P <0 .0 1) ,而且均与同型半胱氨酸水平呈显著正相关 (r分别为 0 .32 ,0 .47,0 .36 ,0 .35 ,P <0 .0 1) ,此相关性在应用逐步回归分析剔除了其他因素后仍然存在。结论 研究表明血清同型半胱氨酸水平是影响颈动脉内膜 -中层厚度、斑块积分及颈动脉顺应性的独立危险因素 ,是影响颈动脉结构和功能的一个重要危险因素。Objective To study the effects of serum homocysteine on carotid artery structures and function. Methods All subjects consisting of 57 coronary artery disease (CAD) patients [(58±8) years] and 24 normal controls [(55±9) years] were verified by selective coronary angiography. Carotid artery parameters (intimal-medial thickness of carotid artery, plaque score, etc) were measured by HP SONOS 5500 echocardiography and calculated by established formulas. All quantitative data were described as mean± sd and analyzed by SAS system (Version 6 12). A value of P <0 05 was consi- dered significant. Results Serum homocysteine level in CAD patients was significantly higher than in normal controls [(17 06±4 54) μmol/L vs (7 89±3 21)μmol/L, P <0 01]. Carotid artery intimal-medial thickness, plaque score, carotid artery stiffness β and Petersons elastic index in CAD patients were significantly higher than those in normal controls ( P <0 01) and all these parameters were positively and independently correlated with serum homocysteine level ( r was, respectively, 0 32, 0 47, 0 36 and 0 35, P <0 01) Conclusions The study demonstrated that serum homocysteine was an independent risk factor having effects on carotid artery intimal-medial thickness, plaque score and carotid artery stiffness. Serum homocysteine level is an independent risk factor that affects carotid artery structures and function.
