摘  要:目的探讨优化术前无症状低钾血症的干预流程对择期手术时间的影响。方法采用历史对照法将患者分为两组,观察组的干预流程为三期四步法,三期即院前防范期、入院监测期、纠正失衡期;四步即采血-送血-检验-查询(借助医院信息系统)。对照组的干预流程为二期四步法,二期即入院监测期、纠正失衡期;四步即采血-送血-检验-问询(借助电话咨询功能)。对比分析两组患者低钾血症与无症状低钾血症的发生率、平均发现时间、平均择期手术时间。结果比较两组患者低钾血症的发生率,观察组明显低于对照组(P<0.01);比较两组患者无症状低钾血症的平均发现时间与平均择期手术时间,观察组明显短于对照组(P<0.01)。结论从院前开始更为全程、有效地优化无症状低钾血症的干预流程,能够为患者早预防、早发现、早处理、早手术创造条件。Objective To investgate the effects of optimized preoperative intervention procedure of asymptomatic hypokalemia on selective operation patients.Methods By retrospective analysis,the observing group's intervention procedure was classified into three phases(prehospital prevention,inhospital monitoring and imbalance correction) and four steps(blood collecting,blood transfusion,blood testing and query by HIS system).The intervention procedure of control group was classified into two phases(inhospital monitoring and imbalance correction) and four steps(blood collecting,blood transfusion,blood testing and query by telephone query function).The prevalence,mean detection time and mean selective operation time were compared and analyzed.Results Comparing with two groups after the treatment,the observer group can decrease the prevalence of hypokalemia,shorten the mean detection time and mean selective operation time of asymptomatic hypokalemia with statistics significances(P<0.01).Conclusion The optimized intervention procedure of asyptomatic hypokalemia,especially beginning from prehospital stage,was more complete and effective to create an optimal condition for patients' early prevention,detection,correction against hypokalemia and receive operation.
