男童血清瘦素水平与肥胖及性发育的关系

摘　　要：目的探讨男童血清瘦素水平与肥胖及性发育的关系。方法选择9～14岁符合单纯性肥胖标准的男性患儿34例为肥胖组,健康男性儿童32例为对照组,分别计算体重指数(BMI)、应用放射性免疫法测定血清瘦素(Leptin)水平、化学发光法测定血清雌二醇(E_2)及睾酮(T)水平,用游标卡尺测量并计算睾丸体积,应用SPSS16.0软件进行组间t检验比较两组间差异,线性相关分析肥胖男童血清瘦素水平与BMI,E_2及T之间的相关性。结果肥胖男童血清瘦素、雌二醇水平及BMI均较对照组儿童高,且差异有统计学意义(P<0.01);肥胖男童血清睾酮水平及睾丸体积均较对照组儿童低,且差异有统计学意义(P<0.01)。肥胖男童血清瘦素水平与BMI呈正相关(r=0.69,P<0.01),与雌二醇呈正相关(r=0.381,P<0.05),与睾酮呈负相关(r=-0.347,P<0.05)。结论肥胖男童存在瘦素抵抗现象,且有性发育不良的趋势。Objective To investigate the relationship between serum leptin level,obesity and sexual development in obese boys. Methods Thirty-four boys aged 9～14 years who were up to the standard of simple obesity were collected as obesity group,32 healthy boys were collected as control group.All subjects were computed for BMI.Their serum leptin level were measured with radioimmunoassay,and estradiol (E_2) and testosterone (T) were determined by the method of chemiluminescence,and the volume of testes were measured by vernier caliper to measure.SPSS 16.0 software was used to deal the data.The t test was used to compare the differences between 2 groups.Linear correlation analysis was used to analyze the relationship between leptin,BMI,estradiol and testosterone.Results The levels of serum leptin,estradiol and BMI of obese boys were much higher than control group,and had the significant differences with control group(P<0.01).The levels of serum testosterone and volume of testes of obese boys were lower than that of control group,and the differences from the control group were statistically significant(P<0.01).The levels of serum leptin of obese boys had the positive correlatiou with BMI(r=0.69,P<0.01), had the positive correlation with E_2 (r=0.381,P<0.05),had the negative correlation with T(r=-0.347,P<0.05).Conclusion Obese boys has leptin-resistant,and has tendency for dysplasia of sexual development.
