腹膜后纤维化致肾盂输尿管积水影像学表现(附5例报告)

摘　　要：目的总结腹膜后纤维化(RPF)并发肾盂输尿管积水的静脉肾盂造影(IVP)、B超及CT表现,以期提高诊断水平。方法回顾性分析经临床病理证实的5例RPF并发肾积水患者的影像学表现。结果5例患者均以腰背部及腹部疼痛、尿路梗阻为首发症状,2例患者IVP检查呈特征性的"三联征"表现,B超检查4例发现不同程度肾盂及输尿管积水扩张,CT平扫表现为腹膜后软组织肿块,增强后4例呈轻到中度强化,1例无明显强化。结论临床上出现腰背疼痛及腹痛,影像学特别是CT检查发现腹膜后肿块及肾盂输尿管积水扩张,应考虑到RPF的可能。Objective To explore the intravenous pyelography(IVP),ultrasonography and CT manifestations in retroperitoneal fibrosis(RPF) in combination of ureterohydronephrosis(UHN) to improve our diagnoses.Methods Retrospective analysis was performed on the imaging manifestations(especially CT) of 5 RPF patients with UHN.Results Lumbago,abdominal pain and urinary obstruction were initial symptoms in all patients,2 of them showed a distinctive triad performance on IVP,varying degrees of UHN were found by ultrasonography in 4,which presented on CT as retroperitoneal soft tissue mass.After enhancement,mild to moderate enrichment was noted in 4,and no obvious in 1.Conclusion Lumbago and abdominal pain are manifested as retroperitoneal mass and UHN on imagings,especially on CT,which should be taken into account the possibility of RPF.
