彩色多普勒超声在深静脉置管血栓形成定期随访中的意义

摘　　要：目的探讨彩色多普勒超声在深静脉置管患者早期预防血栓形成中的应用价值。方法对17例症状检查组患者,即深静脉置管术后出现局部症状申请彩色多普勒超声检查被诊断为血栓形成的病例,采用彩色多普勒超声评价血栓分级情况;对25例随访检查组患者,即深静脉置管术后采用彩色多普勒超声定期随访病例,观察血管内膜是否光滑,血流变化,导管壁是否规整及血栓形成部位、大小、形态、血流通过情况。对比分析两组血栓形成分级情况。结果症状检查组血栓以Ⅲ、Ⅳ、Ⅴ级为主,11例血管横断栓塞面积超过70%,其中7例颈内静脉完全栓塞。随访检查组血栓多以Ⅰ、Ⅱ级为主,仅1例为Ⅲ级血栓。结论对深静脉置管术后患者采用彩色多普勒超声定期随访能早期发现小血栓病变。Objective To discuss the value of regular follow up to detect thrombosis in deep veins on color ultrasonography.Methods Two groups were included in our study.Seventeen cases who had symptoms after deep vein tube placing were proved thrombosis on ultrasonography(Sym group);another 25 cases with deep vein tube placing were regularly followed up to detect the smooth degree of venous inner wall,flow changes,tube wall intact and the position,size,profile and venous canalization,The thrombosis grading were compared in the two groups.Result In the Sym group,the thrombosis stages were mainly Ⅲ,Ⅳ andⅤ and in the regular visit group,the thrombosis was mainly Ⅰand Ⅱ.The difference between the two groups was significant.Conclusions Regular follow up sonographically is important for detecting thrombsis in deep vein after tube placement,because it may help find out minimal thrombi in an early stage.
