控释前列腺素E2栓剂用于足月引产临床分析

摘　　要：目的观察普贝生用于足月引产的临床效果及其安全性。方法将120例妊娠足月、宫颈Bishop评分≤5分、有阴道分娩适应证、无引产禁忌证的单胎头位初产妇随机分为2组:60例将普贝生1枚(10mg)置于阴道后穹窿作为实验组,若无不良反应,12h后取出;60例给予硫酸普拉酮钠200mg/d静脉推注3d作为对照组。比较2组产妇用药前及用药后12h的宫颈Bishop评分、临产情况、剖宫产率、对胎儿及新生儿的影响。结果用药12h后,实验组有51例宫颈评分提高≥2分,有效率为85%;而对照组未次用药12h后有9例宫颈评分提高≥2分,有效率仅为15%。2组间差异有统计学意义(P<0.01);24h内实验组70%(42/60)的孕妇临产,对照组6.6%(4/60)的孕妇临产,2组间差异有统计学意义显著性(P<0.01);实验组65%(39/600)的孕妇经阴道分娩,对照组41.7%(25/60)的孕妇经阴道分娩,2组间差异有统计学意义显著性(P<0.05);2组间胎儿窘迫发生率、新生儿Apgar评分均差异无统计学意义(P>0.05)。结论普贝生能够有效促进宫颈成熟,引产成功率高,不良反应少,可以较安全用于临床。Objective To observe the clinical effects and safety of propess used for induction of term pregnancy.Methods One hundred twenty single term pregnant women with proper fetal position,the cervical Bishop scoring≤5 score,without any vaginal delivery taboo were random divided into two groups.One was the test group,in which one piece of propess was put into the vaginal posterior fornix for 12 hours.Another was the control group,in which the DHA-S was used intravascularly.The cervical maturity(by Bishop scoring)before this management and 12 hours after treatment,the time to labor starting,Cesarean section rate,fetal and neonatal condition were compared between two groups.Results Bishop score of the test group was increased ≥2 points in 85% cases,whereas only 15% cases whose Bishop score increased ≥2 points in control group.The time to labor starting was shortned obviously in test group than that in control group.Cesarean section rate was reduced in the test group.There was no significant difference between two groups in fetal and neonatal condition.Conclusion Propess has no adverse effect to the mothers and babies.It can be used for induction of term pregnancy effectively and sasely.
