摘 要:目的评估不同血液透析方式及不同透析器对β2-微球蛋白(β2-MG)的清除效果。方法测定52例血液透析患者透析前后的β2-MG、尿素氮(Bun)和血肌酐(Scr)的浓度。其中30例应用血仿膜透析器行常规血液透析(HD)治疗;22例应用聚砜膜透析器行血液透析滤过(HDF)治疗。进行透析前后样本均数的比较,计算β2-MG、BUN及Scr的清除率及两种透析方式的KT/V值。结果行HD治疗组β2-MG浓度透后较透前升高,透前(11.80±2.43)mg/L,透后(12.89±2.52)mg/L,(P<0.01);而行HDF治疗组β2-MG透后较透前明显降低,透前(11.30±1.78)mg/L,透后(5.47±2.56)mg/L,(P<0.01);且HDF治疗组β2-MG清除率为51.6%。结论HDF可以较好的清除血液中的β2-MG,减少其在体内沉积,减少透析并发症的发生。Objective To evaluate whether β2-Microglobulin(β2-MG)clearance rate on different hemodialysis modes and dialyzer.Methods Blood urea nitrogen(BUN),Serum creatinine(Scr)and β2-MG were measured before and after a dialysis session in 52 hemodialysis cases :30 on hemodialysis(HD)and 22 on hemodiafiltration(HDF).Their clearance and Kt/V were compared between pre-and post-dialysis in two dialysis modes,respectively.Results The mean post-dialysis β2-MG level was more obvious increase than one of pre-dialysis(12.89±2.52 vs 11.80±2.43,P<0.01)in HD,although the mean post-dialysis β2-MG level was more evident decrease than one of pre-dialysis(5.47±2.56 vs 11.30±1.7 8,P<0.01)in HDF,and the clearance rate of β2-MG was 51.6%.Conclusion HDF can preferably clear β2-MG from blood stream and reduce its sediment,which may decrease occurrences of dialysis complications.
