VVI型起搏器植入术中应用三尖瓣下起搏解决起搏电极常规位置高阈值的临床观察

摘　　要：目的明确三尖瓣下起搏技术在永久起搏器植入术中的应用价值。方法回顾VVI型永久起搏器植入术患者167例,常规位置起搏128例(常规组),常规位置起搏高阈值患者39例,其中21例行三尖瓣下起搏(实验组),观察各组手术时间、起搏阈值、电极阻抗、术后感染、电极脱落、短期起搏阈值升高的差别。结果实验组的患者常规位置和三尖瓣下起搏数据进行成对t检验,结果显示:起搏电压阈值、电极阻抗均有明显下降(P<0.01),实验组三尖瓣下起搏与常规组比较电极阻抗略高(844.38±155.12,728.73±144.49),但绝大多数在允许范围。电极脱落、起搏器囊袋或切口感染、起搏后1个月电压阈值等指标组间差异无统计学意义(P>0.05)。结论三尖瓣下起搏技术是行VVI起搏器常规位置高阈值时可供选择的一个简便、易行、安全、可靠的解决方法。Objective Evaluating the value of sub-tricuspid pacing in the case of elevated threshold on routine position during the implantment of VVI pacemaker. Methods We performed a retrospective study,167 patients were inrolled in our research,the threshold were normal in 128 patients’ routine position(control group),the threshold were elevated in the other 39 patients. Sub-tricuspid pacing were performed in 21 patients(experimental group).The operation duration,voltage threshold,electrode impedance,sac infection,electrode defluvium and voltage threshold ascensus were obtained. Results In experimental group,voltage threshold and electrode impedance were significantly decreased by means of sub-tricuspid pacing (P<0.01),no difference was demonstrated between groups by comparing the sac infection,electrode defluvium and voltage threshold ascensus between control group and experimental group.Conclusion Sub-tricuspid pacing is a safe,convenient way to solve elevated threshold on routine position during the implantment of VVI pacemaker.
