Ⅲ型鼓室成形术在开放式乳突根治术中的应用

摘　　要：目的 了解镫骨插柱听骨链重建Ⅲ型鼓室成形术在乳突术中对听力改善的情况。方法 对1997年～1999年35例乳突根治术中采用镫骨插柱,行听骨链重建手术病人中的29例病人进行了术前、术后对-30天及术后1-2年的听力随访并进行临床分析。结果 术后平均听阈气导平均提高17dBHL。69％病人术后气骨导闭合到20dB,术后1-2年听力保持良好,下降2.8dB,差异无显著性,P>0.05。结论 镫骨插柱在乳突根治听骨链重建术中,听力提高满意,远期疗效稳定。Objective To evaluate the clinical effect and its hearing improvement in shape ossiculoplasty Type Ⅲtympanoplasty in canal wall down mastoidectomy . Methods Twenty - nine cases out of 35 cases who undergone shape ossiculoplasty in canal wall down mastoidectomy during 1997 - 1999 were followed up 1 - 2years. Pure - tone - average (PTA) and Air - bone gap (ABG) including the preoperative , postoperative and 1 - 2years postoperative were analyzed by PEMS version 2.0 statistic software. Results The mean PTA improvement was 17. 6dB. The mean PTA hearing decline in 1 - 2years after surgery was 2.8dB. Almost 69% of ABGs were closed to within 20dB. Conclusion Hearing improvement with a relative stable long - term hearing result is posible with shape piston ossiculoplasty in canal wall down mastoidectomy .
