摘 要:目的 研究不同浓度的葡萄糖-乳酸腹透液和葡萄糖-丙酮酸腹透液对单个核细胞活性和功能的影响,比较他们的生物相容性。方法 应用Ficoll密度梯度离心方法分离6例健康献血者的外周血单个核细胞(PBMC),与不同的液体共同孵育:(1)对照组:RPMI 1640培养液,含10%小牛血清(2)1.5%葡萄糖-乳酸腹透液(3)2.5%葡萄糖-乳酸腹透液(4)4.25%葡萄糖-乳酸腹透液(5)1.5%葡萄糖-丙酮酸腹透液(6)4.25%葡萄糖-丙酮酸腹透液。在37℃、5%CO_2培养箱内分别孵育10分钟、30分钟后,用MTT(亚甲基四唑蓝)法检测单个核细胞活性。并加入10ng/ml的内毒素(LPS)刺激20小时后,应用酶联免疫吸附分析(ELISA)方法检测细胞培养上清液中肿瘤坏死因子α(TNFα人白细胞介素8(I。-8)、可溶性肿瘤坏死因子受体(STNFRI)的浓度。结果PBMC与不同组腹透液共同孵育后,其活性均较对照组显著降低(P<0.05)。在内毒素刺激下产生TNFα、IL-8、sTNFR1的能力受到不同程度的抑制。其中4.25%葡萄糖-乳酸腹透液组抑制作用最显著,1.5%葡萄糖-丙酮酸腹透液组对PBMC产生细胞因子的抑制显著轻于乳酸腹透液组。在同样的缓冲条件下,葡萄糖浓度越高,对PBMC的抑制作用越强。结论 以葡萄糖为渗透剂。Objective To investigate the effect of lactate - buffered and pyruvate - buffered peritoneal dialysate with different glucose concentration on mononuclear leukocyte viability and cytokine production in vitro. Methods Peripheral blood mononuclear cells (PBMC) were isolated from peripheral blood of 6 healthy volunteers by Ficoll density gradient centrifugation. PBMC were incubated with 6 groups of fluids for 10 or 30 minutes: (1) control: RPMI 1640 plus 10% fetal calf serum (2) 1.5% glucose- lactate peritoneal dialysate (3)2.5% glucose - lactate peritoneal dialysate (4) 4.25% glucose - lactate peritoneal dialysate (5) 1.5% glucose - pyruvate peritoneal dialysis fluid ( 6) 4.25% glucose - pyruvate peritoneal dialysate. Cellular viability was assessed by MTT test, then 10ng/ml LPS were added in each fluid to stimulate PBMC for 20 hours, TNFα、 IL- 8、sTNFR1 concentration in the supernatants were measured by ELISA. Results After incubated with different peritoneal dialysate, PBMC viability decreased significantly(P <0. 05), the cytokines release was inhibited compared with control. Moreover, cytokine production after exposure to pyruvate was less inhib-ited than those exposure to lactate.The higher the glucose concentration, the more serious inhibition of PBMC cytokines production. Conclusions High glucose concentration adversely affect PBMC functions. The cytotox-icity of glucose and lactate on PBMC can be minimized by using pyruvate instead of lactate, so it can improve the biocompatibility of peritoneal dialysis fluids using pyruvate.
