结合雌激素治疗功能性子宫出血的临床研究

摘　　要：目的观察结合雌激素用于功能性子宫出血止血的临床疗效及探讨其作用机制。方法12例功能性子宫出血患者予结合雌激素(Premarin)25mg静脉推注,观察其用药前及用药后6 h内每1 h和停药7 d后的出血等级、出血评分及血压、脉搏等变化,以评价其止血疗效.结果12例患者中10例在用药后5～6 h内完全止血,2例患者用药后出血量减少,但未完全止血,但全部患者用药后6 h出血时间均缩短.1例患者用药后出现头晕、心悸,30min后症状消失.停药7 d后随访,10例已止血患者疗效持续.结论结合雌激素注射剂用于功能性子宫出血的止血,疗效好、安全和使用方便,其作用机制可能为缩短出血时间.Objective To observe the clinical effect of Premarin on dysfunctional uterine bleeding (DUB) and to investigate its mechanism. Methods Intravenous injection of Premarin 25mg was gived in 12 patients with DUB .To assess the effect on stop of bleeding the rate of bleeding, the bleeding score , blood pressure and pulse rate were observed before and every hour after injection within 6 hours as well as 7 days after the injections. Results Complete stop of bleeding within five to six hours after Premarin was seen in ten out of twelve patients diminished menstrual flow but not completely stopped was seen in two patients. One patient had dizzy and palpitation after Premarin but felt nothing thirty minutes later. 7 days kter the follow - up of the patients showed that the drug had persistent effect on stop of bleeding. Conclusions Premarin has a good therapeutic, safety and convenient effects on stop of bleeding in patients with DUB.Shorting of the bleeding time might be the main mechanism.
