涉县食管癌的流行情况

摘　　要：目的 报告了涉县自20世纪70年代到新世纪初食管癌患者死亡情况,进一步分析其在人间和空间的分布特点。方法 采用全死因调查和通过三级防癌网的全死因登记报告的方法。结果 20世纪70、80、90年代到21世纪初食管癌死亡率有明显的下降趋势,依次为131.31/10万、104.82/10万、76.31/10万和66.80/10万,趋势检验.X2=204.06,P<0.001,差异非常显著。新世纪初与70年代比较,减少64.51/10万,下降49.12％,u检验P<0.001;食管癌男女性比例70、80、90年代和新世纪初分别为1.8、1.7、1.6和1.7,差别不大;食管癌死亡最小年龄组为30岁组,最高为75-85岁组;食管癌死亡有明显的地理分布差异,清、浊漳河汇合处的夹角地区是食管癌死亡的最高发区。结论 涉县食管癌死亡率从70年代到新世纪初死亡率呈明显的下降趋势,是多年来防治的结果。贲门癌和胃癌则呈上升的趋势,今后的防治策略需要进一步调整。We report the death of esophageal cancer from 1970s to the beginning of new century in order to analyze the sex and age characteristic as well as the geographic distribution. Methods Since the 1970s, all causes of death register system has been established. All the data were checked and analyzed by DIS software . Results There is obviously decreased trend from 70s to the beginning of new century and death rate of esophageal cancer are 131.31/105, 104.82/105, 76.31/105and 66.80/105 respectively. The value of trend test is 204.06, P < 0.0001 with obvious difference. Compared with 70s, it's decreased at the beginning of new century by 64.51/105 , decreased 49.12% and u test P < 0.001; The sex ratio (male to female) are 1.8、1.7、1.6 and 1.7 respectively, no obvious difference; the lowest age group is 30year group, and the highest is 75 - 85 year group; There is obvious geographic distribution. The high death rate of esophageal cancer area is the mingle area where QingZhang river and ZhuoZhang river go together. Conclusion The obvious decreased death rate of esophageal cancer in SheXian from 70s to the beginning of new century is the result of prevention and treatment over years. But cardia cancer and gastric cancer has an increasing trend. So the strategy of prevention and treatment need further adjust.
