摘　　要：目的通过总结涉县29年(1974-2002)来恶性肿瘤的死亡情况,了解各个年代死亡的变化趋势,为恶性肿瘤的防治提供依据。方法采用全死因调查和通过三级防癌网的全死因登记方法。结果从20世纪70年代起至21世纪初,恶性肿瘤的死亡率呈下降趋势,分别为270.04/10万,260.14/10万,211.70/10万和180.11/10万,趋势性检验X2=240.51,P<0.001,统计学有显著性意义;处于前几位的主要恶性肿瘤为食管癌、胃癌、肝癌、肺癌和宫颈癌,以上几种肿瘤之和在各年代分别占恶性肿瘤总死亡的92.13％、91.57％、92.14％和93.90％;男女性比例依次为1.5、1.5、1.7及2.0,有增加趋势;恶性肿瘤的死亡率随年龄的增加而增大;恶性肿瘤死亡有明显的地理分布特点,清、浊漳河夹角处为高发区。结论涉县的恶性肿瘤总死亡率有所下降,主要死因为食管癌、胃癌、肝癌、肺癌等,其中食管癌、宫颈癌死亡率下降比较明显;但胃癌一直居高不降。Objective We described the mortality of malignant tumors in SheXian, Hebei province, China from 1974 to 2002 in orders to analysis the sex and age characteristic. Methods since the 1970s, register system of all causes of death has been established. All the data were checked and analyzed by DIS software. Results The mortality rates of malignant tumors are 272.04/10,000, 260.14/10,000, 211.70/10,000 and 180.11/ 10,000 respectively. The test of trend is 240.51 ( P < 0.001). The main malignant tumors are esophageal cancer, gastric cancer, liver cancer, lung cancer and cervix cancer. The sum of their percentage to all death are 92.13%, 91.57%, 92.14% and 93.90% respectively. The sex ratio (male vs female) are 1.5, 1.5, 1.7 and 2.0 respectively, with a trend of ascend. The mortality rate of malignant tumors increased in accordance with age; there is obvious geographic distribution, and the high incidence area of malignant tumors is the intermingle area of QingZhang river and ZhuoZhang river in SheXian. Conclusion From 1974 to 2002, the mortality rate of malignant tumors has a trend of decrease. In this districe the main cancers are esophageal cancer, gastric cancer, liver cancer, and lung cancer, et al. Since esophageal cancer and cervix cancer have decreased obviously, but the mortality rate of gastric cancer still remained high.
