摘 要:目的探讨痰标本鉴定非结核分支杆菌(NTM)的意义。方法对本院1997年9月~1999年3月255份痰标本鉴定的44株NTM与相对应的37份临床资料对照分析。结果NTM检出率17.3%,NTM对抗结核药总耐药率79.5%。37例NTM检出者抗结核治疗病灶吸收好转率83.3%,24例痰菌阴转率83.3%,反复痰鉴定为NTM者病程长,耐多种抗结核药。结论NTM感染及引发的疾病必须引起医学检验与临床的重视。痰标本一次鉴定的NTM大部分为寄生菌,实验室有必要进一步开展NTM的菌种鉴定技术。连续痰鉴定为NTM者为NTM肺病高度可疑者。诊断多耐药肺结核,必须除外NTM肺病。Objective To study the significance of identifying nontuberculous my-cobacteria(NTM)in sputum.Methods To analyze comparatively44strains of NTM which having been identified in sputum from September1997to March1999with37corresponding clinical data.Results The positive rate of NTM examined in all age groups was17.3%.In44patients,79.5%were with anti-tuberculous-drug-resistance,In37with lung lesions83.3%had lesions absorbed after anti-tuberoulous treatment,while24with tuberculous bacterium in sputum83.3%turned negative.Those patients with NTM identified repeatedly in sputum proved to be long-term sufferers and resistant to multiple anti-tuberculou-drugs.Conclusions The NTM infection and related diseases should arouse attention of laboratory and cliniacl staff.The majority of NTM once identi-fied in sputum may be parasitic bacteria.We suggest it's necessary to develop the tech-nique to indentify NTM bacteral species in laboratory.Those with sputum NTM identified in succession must highly suspected of pulmonary NTM disease.The diagnosis of multi-ple-drug-resistant pulmonary tuberculosis should rule out NTM pulmonary disease.
