血清GPDA、AFu活力联合检测对原发性肝癌的诊断价值

摘　　要：目的评价血清甘氨酰脯氨酸二肽氨基肽酶(GPDA)、α-L-岩藻糖苷酶(AFu)联合检测在原发性肝癌诊断中的临床意义。方法收集42例健康人血清和78例原发性肝癌(PHC)及150例非PHC患者的血清。分别测定GPDA、AFu、AFP含量,以GPDA≥136U/L、AFu≥172nkat/L(均以健康组x±3S)为PHC阳性预示值。AFP≥400ng/ml为PHC预示值。结果PHC患者血清GPDA(156.2±39.2),AFu262.0±88.6均显著高于健康对照组(P<0.001)。对PHC的诊断阳性率GPDA达到64.1%,AFu达到75.6%,AFP仅为59%,若以其中二项或三项阳性作为诊断PHC,则阳性率为89.7%,特异性为91.9%。结论血清GPDA。Objective To evaluate the clinical significance of glycyl proline dipetidyl aminopeptidase(GPDA)andα-L-fucosidases(AFu)in diagnosing primary hepatic carcinoma (PHC).Method GPDA,AFu and AFP in78serum samples from patients with primary nepatic Carcinoma,150from patients with other diseases but not hepatic carcinoma and42from healthy people were detected respectively,taking GPDA≥136U/L,AFu≥172nkat/L and AFP≥400ng /ml as the positive value for pre-dicting PHC.Re sults The level of serum GPDA and AFu in patients with primary carcinoma of liver were156.2±39.2and262.0±88.6respectively,significantly higher than those in healthy control(P<0.001).the positive rate of GPDA,AFu and AFP in-creased to64.1%,75.6%and59.0%respectively.Taking two or three of these posi-tive items together as diagnostic standard,would raise the positive rate to89.7%and the specifivity to91.9%.Con clusions Combined detection of GPDA and AFu in serum will significantly raise the positive rate in diagnosing PHC.
