摘　　要：目的研究T淋巴细胞亚群在乙肝相关疾病患者外周血中的变化及其临床意义,并探讨乙肝相关疾病患者血中乙肝病毒脱氧核糖核酸(HBV-DNA)含量与T淋巴细胞免疫的关系。方法应用流式细胞仪检测乙肝病毒携带者(ASC)30例,慢性乙型肝炎(CHB)31例,乙肝肝硬化(LC)28例,原发性肝癌(PHC)15例及健康对照30例的外周血T淋巴细胞亚群,采用荧光定量聚合酶链反应(FQ-PCR)检测上述104例乙肝相关疾病患者血清中HBV-DNA含量,并进行统计分析。结果与对照组比较,ASC、CHB及LC组的CD3+、CD4+、CD4+/CD8+、CD56+显著性降低,分别为P<0.05、P<0.05、P<0.01、P<0.05;PHC组CD4+、CD4+/CD8+均有显著性升高(P<0.01),CD8+无统计学意义。对照组、HBV-DNA阳性组和HBV阴性组三组相比,CD3+、CD4+、CD8+、CD4+/CD8+等各项指标差异均有统计学意义(P<0.01)。结论乙肝相关性疾病患者细胞免疫功能紊乱,且与疾病发展有一定关系,外周血T淋巴细胞发生改变与HBV-DNA含量有关系。Objective To observe the change of the peripheral blood T lymphocyte subsets in patients with Hepatitis B related diseases,and to observe the correlation between HBV-DNA and cellular immune function of T lymphocyte in patients with Hepatitis B related disease.Methods We tested the peripheral blood T lymphocyte subsets with flow cytometry in 30 patients with Hepatitis B carriers(ASC),31 patients with chronic Hepatitis B(CHB),28 patients with liver cirrhosis(LC),15 patients with primary liver cancer(PHC) and 30 heathy individuals,and test the contents of HBV-DNA with FQ-PCR in 104 patients with Hepatitis B related disease,results were compared and analyzed among these groups.Results Compared with normal controls,the number of CD3+、CD4+、CD56+ and the ratio of CD4+/CD8+ significantly decrease(p<0.05、p<0.05、p<0.01) in patients with ASC、CHB and LC,the numbers of CD4+ and the ratio of CD4+/CD8+ were higher(p<0.01).The number of CD3+、CD4+、CD8+ and the ratio of CD4+/CD8+ were significantly different(p<0.01) between control group、HBV-DNA positive group and HBV-DNA negative group.Conclusion Abnomality of cellular immunity exists in the patient with Hepatitis B related disease,and relates to the development of the disease.The change of peripheral blood T lymphocyte is related to the concentration of HBV-DNA.
