妊娠期高血压疾病肝功能、血小板变化及发生HELLP综合征的处理

摘　　要：目的:分析妊娠期高血压疾病对肝功能、血小板变化的影响,探讨HELLP综合征的发生和终止妊娠的时机、方式及对母儿的影响。方法:对我院2001-01～2009-01住院分娩的532例妊娠期高血压疾病患者资料进行回顾性分析。结果:重度患者分娩孕周平均35±2.2周,有98例采用剖宫产终止妊娠,剖宫产率66.7%。轻度患者分娩孕周平均38±2.5周,有115例采用剖宫产终止妊娠,剖宫产率29.9%。随病情加重,肝功能异常患者增多,血小板计数则随病情进展而减少。重度患者HELLP综合征发生率为10.2%,其中剖宫产12例,剖宫产率80%。轻度患者无HELLP综合征发生。结论:随着妊娠期高血压疾病病情的加重,出现肝功能异常明显增多,表明对肝脏的损害也逐步加重。血小板计数则随病情进展而减少。重度患者发生HELLP综合征严重影响母婴预后,通过及早诊断、积极处理,适时以剖宫产作为主要的终止妊娠的手段,可以获得较好的母婴结局。Objective:To analyse the effection of the Hypertension disorder complicating pregnancy brings to the liver fuction and Platelet,investigate the relationship between the happening of the HELLP syndrome and the time 、the method of pregnacy termintion prognosis in pregnancy.Methods:Retrospective analyses were done on 532 patients with pregnancy termination from Janauary 2001 to Janauary 2009 year.Results:The severe patients have 35±2.2deliver`s weeks,98 patients underwent cesarean section(66.7%).the mild patients have 38±2.5 deliver`s weeks,115 paitents underwent cesarean sections(29.9%).As the patient's condition gets worse,the patients with unusual liver fuction will be much more,while the total of the platelets becomes lower with the development of the disease condition.The severe patient's HELLP syndrome taken place rate is 10.2%,among them,there were 12 patients underwent cesarean sections(80%).the mild patients have no syndrome.Conclusions:As the condition of the Hyppertension disorder complicating pregnancy becoming worse,the patients with unsual liver fauction obviously getsmore,which indicates that the bad affection to the liver becomes much more heavier,while the totle of platelets gets lower.The severe patients who gets HELLP syndrome would have a serious influence in mother and perinatal fetuls.Diagnosis as soon as possible,active dealing,timely teminating pregnancy using cesarean section as the first choice.Above these,both mother and perintal fetus would have a good result.
