糖尿病视网膜病变光凝术后中央动脉血流动力学变化

摘　　要：目的观察糖尿病视网膜病变(diabetic retinopathy,DR)行全视网膜激光光凝术后视网膜中央动脉(central retinal artery,CRA)的血流动力学改变情况。方法应用彩色多普勒成像(color doppler imaging,CDI)检测152例增殖期DR激光光凝术后患者的CRA收缩期血流峰值速度(peak systolic velocity,PSV)、舒张末期血流速度(end diastolic velocity,EDV)、阻力指数(resistance index,RI)及视网膜中央静脉(central retinal vein,CRV)的最大血流速度(maximum blood flow velocity,Vmax)、最低血流速度(minimum blood flow velocity,Vmin),并作对比分析。结果 CRA的PSV、EDV在全视网膜激光光凝术后有所增加,RI降低;CRV的Vmax、Vmin均有所降低,且与光凝术前比较差异无统计学意义(P>0.05),而光凝组术后与对照组1个月后比较差异有统计学意义(P<0.05)。结论全视网膜激光光凝术是治疗增殖期DR的一种安全有效方法,光凝术后血液循环障碍依然存在;应用CDI测定光凝术后患者CRA的血流动力学改变具有一定的临床价值,可为DR的诊断和治疗提供依据。Objective To investigate the ocular hemodynamics changes of central retinal artery in patients with diabetic retinopathy(DR) after pan retinal photocoagulation.Methods The peak systolic velocity (PSV),the end diastolic velocity (EDV) and the resistance index (RI) of CRA,the maximum blood flow velocity(Vmax) and the minimum blood flow velocity(Vmin) of central retinal vein in 152 patients with diabetes retinopathy ( DR ) after pan retinal photocoagulation were determined with color doppler imaging (CDI),the clinical outcomes were obtained and compared.Results Increased blood flow velocity and reduced resistance in CRA and reduced blood flow velocity in central retinal vein were found in patients with diabetes retinopathy (DR) after pan retinal photocoagulation.There was no statistically significant difference ( P > 0.05) between before and after pan retinal photocoagulation,but there was statistically significant difference(P <0.05) between photocoagulation group and control group one month after pan retinal photocoagulation.Conclusion It appears that pan retinal photocoagulation is an effective and safe procedure for the treatment of proliferative diabetic retinopathy,the circulatory disturbance of CRA still exists in patients with DR after pan retinal photocoagulation; Measurement of ocular hemodynamics in the central retinal artery with diabetic retinopathy after pan retinal photocoagulation using CDI has some clinical significance and can offer evidences for the diagnosis and treatment of diabetic retinopathy.
