聚丙烯网片在盆底重建术中的应用(附18例分析)

摘　　要：目的初步探讨聚丙烯网片在女性盆底功能障碍性疾病手术重建盆底功能中的应用。方法选择妇科2007年7月-2008年2月因盆底功能障碍应用聚丙烯网片进行全盆底重建患者18例,年龄50～71岁,平均(59.92±7.33)岁。阴道前壁膨出18例,子宫脱垂11例,阴道后壁膨出14例,阴道残端膨出7例。其中7例既往有子宫切除及阴道前壁和(或)后壁修补术史。结果 18例患者总手术时间平均为(1 50.0±42.1)min(100～225min),其中放置补片的时间平均21.3min。术中出血平均(255.6±97.9)mL(100～500mL),术中未发生周围脏器及重要血管神经的穿刺损伤。住院时间(7.61±1.3)d(6～11d)。平均随访(14.3±2.3)个月(11～18个月),除1例阴道顶端Ⅰ度膨出复发,2例网片暴露和1例患者诉术后性生活不适外,其他患者恢复良好。结论聚丙烯网片在女性盆底重建手术中的应用,方法简单,短期随访并发症少,复发率低。Objective To investigate the application of polypropylene mesh in female pelvic reconstruction surgery.Methods Eighteen patients were undergone the operation for pelvic organ prolapse who were admitted from July 2007 to February 2008.Mesh was used for pelvic floor reconstruction in the operation.The average age was(59.92±7.33)years old.The number of the patients with the anterior vaginal wall prolapse was 18 cases,with the uterine prolapse was 11 cases,with the posterior vaginal wall prolapse was 14 cases,with the vigina-stub prolapse was 7 cases.Seven patients had done hysterectomy and anterior and/or posterior colporrhaphies before. Results The mean operation time was(150.0±42.1)min(100～225 min).The mean time of mesh placement was about 21.3 min.The mean hemorrage was(255.6±97.9)mL(100～500 mL).All the patients were well-being after operation.There were no important organs,blood vessels and nerves injuried.The mean length of stay was(7.61±1.3)d(6～11 d).The mean period of follow-up after operation was(14.3±2.3)months(11～18 months).Among the patients,one cases was stage Ⅰ recurrence.One case was uncomfortable in sex life.And the exposure of mesh was happened in 2 cases.Conclusion The application of mesh in female pelvic reconstruction surgery is easy and simple.And the short term outcome of mesh in pelvic floor reconstruction is with lower recurrence and little complications.
