159例屋尘螨变应原结膜激发试验的临床特征分析

摘　　要：目的探讨屋尘螨变应原结膜激发试验的临床特点。方法对159例变态反应病患者进行屋尘螨变应原结膜激发试验,将12项眼部症状及体征作为观察指标量化,评分后分别记录并统计整理分析。同时进行屋尘螨变应原皮肤点刺试验(skin prick tests,SPT)以及血清屋尘螨变应原特异性免疫球蛋白E(serum specificimmunoglobulin E,d1-sIgE)检测。结果屋尘螨变应原结膜激发试验最常见症状体征为睑结膜充血(81.1%),球结膜充血(74.2%),球结膜水肿(20.8%),其评分分值与皮肤点刺结果有显著相关(P<0.01)。159例患者中无1例出现畏光及视力下降。结论屋尘螨变应原结膜激发试验是模拟自然状态,以结膜充血为主要临床特点,球结膜水肿的出现在评判屋尘螨变应原结膜激发试验结果上占有重要地位。Objective To investigate the clinical features of allergen conjunctival provocation tests with Dermatophagoides pteronyssinus(Der p).Methods One hundred and fiftynine allergic outpatientes who were examed by allergen conjunctival provocation tests with Der p were observed,and their twelve clinic symptom and physical sign scored according to their extent were analysed.Skin prick tests(SPT)and serum specific immunoglobulin E(d1-sIgE) determinations were detected at the same time.Results The most common clinic symptom and physical sign was eyelid conjunctival conjestion.The next,was ball conjunctival conjestion.The rate of ball conjunctiva chemosis was only 20.8%,but its score was strongly related with the result of SPT(P<0.01).Conclusion Allergen conjunctival provocation test imitates nature allergic disease,charactered with conjunctival conjestion.The appearance of ball conjunctiva chemosis plays an important role in the judgement of allergen conjunctival provocation tests.
