摘　　要：目的探讨标准外伤大骨瓣开颅术救治重型颅脑损伤的临床效果。方法采用美国标准外伤大骨瓣开颅术对78例重型颅脑损伤患者进行手术治疗,并观察其治疗效果。结果按照GOS标准,伤后半年随访:预后较好35例(45%),其中恢复良好23例,中残12例;31例预后较差(40%),其中重残21例,植物状态10例;死亡12例(15%)。结论标准外伤大骨瓣减压术可明显改善重型颅脑损伤患者的预后,降低病死率,提高生存率。Objective To explore the clinical effect of standard trauma large bone flap pressure reduction operation for severe traumatic brain injury.Methods The USA standard trauma large bone flap was adopted to cure severe traumatic brain injury of 78 cases,then the clinical effect of treatment was obserued.Results According to Glasgow outcome scale standard,a follow-up was con ducted half a year later,which indicated that 35 cases got favorable outcome(45%),including 23 with good recovery and 12 mederate deficit,31 cases got unfavorable outcome(40%),including 21 with severe deficit and 10 living in plant status,and 12 cases were dead(15%).Conclusion Applying standard trauma large bone flap pressure reduction operation can greatly improve the prognosis of severe traumatic brain injury patients,reduce death rate,and improve patients' living rate.
