青年军人基础心率的调查及分析

摘　　要：目的 为明确青年军人基础心率(BHR,即清醒静息心率)的分布、正常范围,窦性心动过缓(SB)、显著性窦性心动过缓(OSB)的现患率及相关因素。方法 采用普查法测量江西省某军队院校青年学员(a组,1570人)和分层整群随机抽样方法测量部分普通驻军官兵(b组,1165人)共2735人(a组+b组)的基础心率。结果 青年军人BHR的分布不符合正态分布规律。a组+b组、a组、b组BHR均数分别为(63.55±9.9)bpm、(62.0±8.8)bpm和(66.0±7.5)bpm;中位数分别为63.0bpm、60.4bpm和66.1bpm;正常值参考范围(中位数法)分别为47～83bpm、45～88bpm和47～84bpm。SB与OSB现患率分别为32.3％和4.9％,45.5％和6.9％,14.5％和2.2％。相关分析示最佳训练成绩、训练月数、军龄与BHR负相关。结论 青年军人中BHR较低,SB和OSB现患率均较高。这不仅与青年军人训练强度高有关,而且与其训练持续的时间有关。Objectives To determine the normal range and mean value of basic heart rate (BHR, i.e. rest and awaken heart rate), the prevalence rates of sinus bradycardia (SB) and obvious sinus bradycardia (OSB) of basic heart rate of youths in military service, and to analyze the influence factors to each item listed above. Methods Two thousand seven hundred and thirty five youths in military service aged 18-32 years (proup a + b) were involved. 1 570 military school students (group a ) and 1 165 ordinary military youths (group b) were investigated by measuring BHR and answering the questionnaires by way of overall survey and cluster random sampling. Results BHR of youths in military service didn't coincided with normal distribution. The mean values of BHR in the group a + b, group a and group b were 63. 5±9. 9bpm, 62. 0±8. 8bpm and 66. 0±7. 5bpm while the median values were 63. 0bpm, 60. 4bpm, 66. 1bpm respectively. And the normal ranges (estimated by P2. 5-P97. 5) were 47-83bpm, 45-88bpm, and 47-84bpm respectively. The prevalence rates of SB and OSB were 32.3% and 4. 9%, 45.5% and 6.9%, 14. 5% and 2. 2% respectively. The results obtained by correlation analysis revealed that the factors as training years and scores were negatively correlated with BHR. Conclusions The military youths with low BHR level may have high prevalence rate of SB and OSB. This fact may be related to both the training intensity and the length of training course.
