腹腔镜治疗小儿腹股沟斜疝的临床研究(附2880例报告)

摘　　要：目的评价腹腔镜下疝囊高位结扎术的临床治疗效果,探讨术后常见并发症的原因和处理。方法回顾性分析2004年1月～2010年1月实施腹腔镜小儿腹股沟疝囊高位结扎术的2880例患儿的临床资料。结果本组所有患者手术均顺利,术中无中转开腹,出血少。单侧手术时间5～8min(中位时间7分钟),双侧手术时间12～16min(中位时间14分钟),术中发现对侧鞘状突未闭578例(23%)。术后并发症32例(1.0%),均得到及时处理痊愈。住院3～4d。随访时间6月至3年,25例复发(0.8%)。结论应用腹腔镜疝囊高位结扎治疗小儿腹股沟疝是安全可行的,且创伤小,手术时间短、副损伤小、恢复快、住院时间短,并可以发现并处理隐性鞘状突未闭,值得临床推广。Objective To evaluate the clinical therapeutic efficacy on high ligation of hernial sac in children by laparoscope,and to discuss the causes and treatment of common postoperative complications.Methods From Junuary 2004 to Junuary 2010,the clinical data of 2880 chidren cases with indirect ingunal hernia received laparoscopic high ligation of hernial sac were analyzed retrospectively.Results The operations of all cases were performed smoothly.No case was conversed to open surgery.The unilateral operative time ranged from 5 to 8 minutes(average 7 minutes).Bilateral operative time ranged from 12 to 16 minutes(average 14 minutes).The ipsilateral Persistence of processus vaginalis peritonei was found in 578 cases(23%) during the operation.Postoperative complications occurred in 32 cases(1%),which were cured via intime management.The hospitalization time was in 3～4 days.The follow-up time ranged from 6 months to 3 years.Relapse was found in 25 cases(0.8%).Conclusion Laparoscopic high ligation of hernial sac is safe and feasible for the treatment of indirect inguinal hernia in children.It possesses less trauma,shorter operating time,fewer postoperative complications,quicker recovery and shorter hospitalization time.And it can find out and treat on latent persistence processus vaginalis peritonei.
